| 2 | Helicobacter pylori (H. pylori) is a bacterium that infects the stomach lining. It can cause chronic gastritis, peptic ulcers, and over many years can increase the risk of certain stomach cancers.
🔹 How infection occurs

➟ Usually spreads through oral–oral or faecal–oral contact
➟ Contaminated food or water may contribute
➟ Infection is often acquired during childhood
➟ Household transmission can occur
➟ Many infected people remain asymptomatic for years

🔹 Common signs

➟ Burning or aching pain in the upper abdomen
➟ Bloating or early fullness
➟ Nausea
➟ Frequent belching
➟ Reduced appetite
➟ Symptoms may worsen when the stomach is empty
➟ Some people have no symptoms at all

➟ Urea breath test is commonly used for active infection
➟ Stool antigen test can also detect active infection
➟ Endoscopy with biopsy may be needed if alarm symptoms are present
➟ Blood antibody tests are less useful for confirming current active infection because antibodies can remain positive after treatment

➟ Antibiotics and bismuth can also cause false-negative results
➟ Medication timing before testing should follow clinician or laboratory instructions

| 6 | 🔥 ما هي قرحة الفراش؟
هي تلف موضعي في الجلد والأنسجة تحته، يحدث بسبب الضغط المستمر (Pressure)، أو القص (Shear)، أو الاحتكاك (Friction)، مما يقطع التروية الدموية عن المنطقة و يؤدي إلى نقص التروية (Ischemia) ثم النخر (Necrosis).
🧪 تقييم الخطر: مقياس برادن (Braden Scale) - لا تتجاهله أبداً!
هو مقياس عالمي لتقييم خطر الإصابة بقرح الفراش، ويقوم على 6 معايير:
1. الإدراك الحسي (Sensory Perception): هل المريض يشعر بالضغط؟
2. الرطوبة (Moisture): هل جلده مبلل بالعرق أو البول؟
3. النشاط (Activity): هل يتحرك أم طريح الفراش؟
4. الحركة (Mobility): هل يستطيع تغيير وضعه؟
5. التغذية (Nutrition): هل يتناول كفايته من البروتين والسعرات؟
6. الاحتكاك والقص (Friction & Shear): هل يُسحب على الملاءات؟
النتيجة: كل معيار من 1-4 (ما عدا الاحتكاك من 1-3).
🔴 ≤ 18 = خطر مرتفع → يجب تفعيل بروتوكول الوقاية فوراً!
🔴 ≤ 9 = خطر شديد جداً → يحتاج عناية فائقة وعنوان استشارة.
🩺 مراحل قرح الفراش (Staging) - احفظها لتوثيق دقيق:
🔹 المرحلة 1 (Stage 1): احمرار لا يختفي عند الضغط (Non-blanchable Erythema) على جلد سليم.
🔹المرحلة 2 (Stage 2): فقدان جزئي للجلد (Partial-Thickness) يشمل الأدمة (Dermis)، يظهر كجرح سطحي أو فقاعة (Blisters).
🔹المرحلة 3 (Stage 3): فقدان كامل لسماكة الجلد (Full-Thickness) مع رؤية الدهون تحت الجلد (Subcutaneous Fat) ولكن دون عظم أو وتر.
🔹المرحلة 4 (Stage 4): فقدان كامل للجلد والأنسجة مع رؤية العظم، الوتر، أو العضلة (Bone, Tendon, Muscle).
🔹غير قابلة للتصنيف (Unstageable): الجرح مغطى بنسيج ميت (Slough or Eschar) يمنع رؤية العمق.
🔹إصابة الأنسجة العميقة (Deep Tissue Injury - DTI): بقعة أرجوانية أو كستنائية اللون على جلد سليم، تشير إلى تلف عميق في الأنسجة.
🛡️ الخطة الذهبية للوقاية
(The Golden Prevention Bundle):
1. إعادة الوضعية (Repositioning):
حوّل المريض كل ساعتين (q2h) على الأقل، باستخدام وسادة أو مناورة الرفع (Lift Sheet) وليس السحب. تجنب الضغط على المناطق الحمراء.
2. الأسطح الداعمة (Support Surfaces): استخدم مرتبة ضغط متناوب (Alternating Pressure Mattress) أو مرتبة الهواء (Air Mattress) للمرضى ذوي الخطورة العالية.
3. العناية بالجلد (Skin Care): حافظ على نظافة الجلد وجفافه. استخدم غسول لطيف (pH-Balanced Cleanser) و كريم حاجز (Barrier Cream) للمناطق المعرضة للرطوبة.
4. التغذية (Nutrition): اطلب استشارة أخصائي تغذية. تأكد من كفاية البروتين (1.2-1.5 g/kg/day) و السوائل و فيتامين C و الزنك لتسريع الشفاء.
5. الحركة المبكرة (Early Mobilization)
شجع المريض على الحركة قدر الإمكان، حتى لو كان مجرد تحريك الأطراف في السرير.
6. الفحص اليومي
(Daily Skin Assessment):
افحص الجلد من الرأس إلى القدمين (Head-to-Toe) عند كل وردية، وركز على المناطق البارزة (الكعب، العجز، الورك، الكتف، الأذن).
⛔ أخطاء تمريضية قاتلة
(Fatal Nursing Errors):
❌ لا تتدليك (Massage) المنطقة الحمراء أبداً! التدليك يزيد الضغط على الأوعية الدموية المتضررة ويفاقم النخر.
❌ لا تستخدم حلقة القطن (Donut Cushion) للجلوس، لأنها تسبب ضغطاً على محيط المنطقة وتزيد نقص التروية.
❌ لا ترفع رأس السرير أكثر من 30 درجة دون داعٍ، لأن ذلك يزيد قوى القص (Shear) على العجز (Sacrum).
❌ لا تتجاهل الاحمرار البسيط، فهو إنذار مبكر لقرحة قادمة. | 79 |
