VIEW POINT: Ever since the Cass Report was published in April 2024 attempts have been made to circumvent its essential findings, that social transitioning is ‘not a neutral act’ and the recommendation in favour of an ‘abundance of caution’. The most serious attempt to bypass this report has recently been stopped. Kings College Hospital in London was due to start a ‘study’ into the effects of prescribing puberty blockers to adolescents. The study had commenced recruiting children with gender dysphoria and many medical observers described it as nothing more than a back door attempt to give puberty blockers to children. The study was heavily supported by transgender advocacy organisations. It was called the Pathways study and it has been blocked by the UK’s Medicine and Health Care Products Regulatory Agency, the MHRA due to safety concerns. This is the role of the MHRA, the question must be asked how did this study get so far as to be able to start recruitment. It has all the hallmarks of a rushed botched piece of work that is unworthy of medical research as it is totally agenda driven. The MHRA stopped the study going ahead for a number of serious scientific issues with study design and the unknown harm that may be caused to young people prescribed these drugs such as bone density and fertility. The MHRA flagged the ‘significant and as of yet unquantified risk of long term biological harms’. This trial is stopped, the scientific gap highlighted by the MHRA is unbridgeable. There were also serious ethical concerns. Each participant would receive a £30 voucher for each of the 15 cognitive assessment sessions and the same for each of the MRI scans over the course of the trial. This amounts to the bribery of the vulnerable. It puzzles me how such a scheme ever got approval from Kings Hospital ethical committee. Clearly this is a study unlike any other piece of research and it should never have been allowed to progress as far as it did. Children from Northern Ireland were due to participate but the health minister Mike Nesbitt has wisely stopped this happening in the light of the wider concerns raised. He was simply following the best practice advice offered by both the MHRA and the Cass Report. While his decision is commendable it should not be controversial due to the medical science. Diana Dodds MLA stated that ‘children in the United Kingdom are safer’. Those of us who put science above ideology should have no difficulty in agreeing with that. Sinn Féin are not happy. First Minister of the Northern Ireland Assembly Michelle O Neill described health minister Nesbitt’s decision to stop the Northern Ireland portion of the study as ‘disgraceful’. Despite all the medical, scientific and ethical red flags, despite independent scientific advice Sinn Féin believes that the study should go ahead. This is in the full knowledge that serious harm will be caused to children and that best practice guidelines give no credible reasons to proceed. It is therefor fair and reasonable to surmise that Sinn Féin have made the very conscious decision to place ideology above medical science and if that means causing harm to children so be it. Sinn Féin is supportive of the prescribing of puberty blockers in both parts of Ireland. Therefor it follows that vulnerable children with gender dysphoria are at risk of harm wherever Sinn Féin has influence which currently is nowhere but of course the worry is that the political landscape could change.Nothing is certain in politics. Politicians in the Republic of Ireland have a deplorable history of sacrificing science for ideology. When the Green Party was entering the coalition government of 2020 and the programme for government was being crafted they insisted that transgender care should be delivered through the WPATH pathway, an agenda driven ideologically harmful pathway, both Fianna Fáil and Fine Gael acquiesced to obtain power. The welfare of young people with gender dysphoria was secondary to obtaining power. We must be vigilant that ideology and politics should never corrupt medical care.
