#Disease control & prevention key exam points 1. Prevent risk factors from developing; start in childhood. Examples: Healthy childhood habits + supportive social and economic environment. 2. Primary prevention — منع حدوث المرض Before disease onset: reduce existing risk factors. Two modes: Health promotion + Specific protection. • Health promotion: Healthy diet, physical activity, health education. • Specific protection: Vaccination, chemoprophylaxis, specific nutrients. A. COVID vaccination = Primary prevention; its mode = Specific protection. Specific protection. Screening for obesity to prevent type 2 diabetes = Primary prevention of diabetes. Primary prevention is generally the most effective and economical long-term approach in this lecture. Aim: Arrest disease progression and prevent complications. Examples tested: Early detection of diseases complicating pregnancy; breast tumour detection. Searching the submerged part of the iceberg = Secondary prevention. Treating an infectious case = Secondary for the patient, Primary for contacts. • Disability limitation: Prevent muscle atrophy after stroke. • Rehabilitation: Physiotherapy to improve movement after stroke. Care for established organ damage after COVID = Tertiary prevention. 5. Quaternary prevention — الحماية من التدخلات الزائدة Prevent overmedicalization, overdiagnosis and overtreatment.

• Collective risks: air pollution, poor housing, poor water supply and poor healthcare services. ⑥ Psychosocial environment and agent types • Psychosocial environment = culture, beliefs, customs, habits, education and social relationships. • Psychosocial factors can HELP or HARM health. • Mental disorders are not synonymous with psychosomatic disorders. • Night blindness + dry conjunctiva + corneal changes → vitamin A deficiency → NUTRITIONAL agent. @OSOS_team

#Concept of Disease — Key exam points ① Illness vs Disease • Illness = SUBJECTIVE experience of feeling unwell; • Germ theory: a specific microbe causes a specific disease. • Epidemiological triad = AGENT + HOST + ENVIRONMENT. • TB example: bacillus + susceptible host + overcrowding/poor ventilation. • The triad is less suitable for complex NCDs without one clearly defined agent. • Multifactorial causation = many contributing factors; web of causation = their complex interrelationships. • Prevention does NOT require removing ALL contributing factors. ③ Natural history of disease — أكثر جزء تكرر • Natural history = disease progression from initiation to recovery, disability or death WITHOUT prevention or treatment. • Each disease has its own natural history; it is best studied through COHORT follow-up. • Prepathogenesis = before the disease process begins; risk conditions exist. • Agent, host and environment being present is NOT enough; their interaction initiates disease. • Pathogenesis = disease process has started; changes may precede symptoms. • Chronic disease changes are often gradual and less obvious than acute infections. • Prepathogenesis → PRIMARY prevention; pathogenesis → SECONDARY and TERTIARY prevention. • Primordial prevention prevents the emergence of risk factors. ④ Iceberg phenomenon — الظاهر والمخفي • Tip above water = clinically apparent / diagnosed cases. • Submerged portion = latent, inapparent, presymptomatic, undiagnosed cases and carriers. • Waterline = boundary between apparent and inapparent disease. • 30 known hypertension cases + 120 newly detected by screening = ICEBERG phenomenon. ⑤ Risk and risk factors • Risk = probability of an adverse outcome; risk factor = an attribute/exposure associated with disease. • Risk factors should be identifiable BEFORE disease onset; association does not automatically prove causation. • Presence does not guarantee disease; absence does not guarantee protection. • Modifiable: smoking, hypertension, high cholesterol, inactivity, obesity. Immutable: age, sex, genetic/family factors.

#Analytic@OSOSCM1 • Case-control: select people WITH and WITHOUT the disease, then compare previous exposures. • Cohort: classify by exposure, then follow for NEW outcomes. • For incidence of a first event, participants must initially be free of THAT outcome. • Rare DISEASE → case-control; rare EXPOSURE → cohort. ③ Cases, controls and matching • Cases have the disease; controls do NOT have the disease under study and should come from the same source population. • Define diagnostic and eligibility criteria beforehand; select representative cases and controls. • The lecture favors newly diagnosed cases over old prevalent cases. • Matching makes cases and controls similar in potential confounders, such as age and sex. • Pair matching = match each case individually; frequency matching = match group proportions. • Random selection is NOT random assignment to treatment. ④ Bias and confounding • Confounding: a third factor is associated with exposure and independently affects the outcome. • Recall bias: cases and controls remember previous exposures differently. • Selection bias: selected cases or controls do not represent the intended source population. • Berkson bias: hospital selection/admission creates a distorted association. • Interviewer bias: questioning differs according to disease/exposure status. • Attrition = loss to follow-up; it can bias cohort results.

• A case series may later supply the CASE group for a case-control study. ⑥ Ecological / correlation studies • Unit of analysis = GROUPS or POPULATIONS, not individual people. • Examples: meat consumption ↔ colon cancer; smoking ↔ lung cancer; cigarette sales ↔ CHD mortality. ⑦ Person, place and disease patterns • Person: age, sex, occupation, social class, habits and stress. Place: countries, regions, urban/rural areas. • Rural infant and maternal mortality may be HIGHER because access to care can be poorer. @OSOS_team

#Descriptive Epidemiology — Key exam points ① الفكرة الأساسية واستخداماتها • Descriptive epidemiology describes disease FREQUENCY and DISTRIBUTION by TIME, PLACE and PERSON. • Descriptive → generates hypotheses; analytic → tests hypotheses; experimental → confirms hypothesis . • Uses: measure the problem, describe patterns and trends, detect outbreaks, guide planning and evaluation. ② Procedures — ترتيب خطوات الدراسة • Define population → define disease → describe time/place/person → measure disease → compare with known indices → formulate a hypothesis. • A defined population provides the denominator; an operational case definition provides measurable criteria. Random assignment of an intervention is NOT a descriptive-study procedure. ③ Time trends — أكثر الفروق اللي تتلخبط • Short-term fluctuations → changes over a brief period, such as an epidemic. • Seasonal trends → repeat during particular seasons. • Cyclic trends → repeated rises and falls at recurring intervals. • Historical cyclic examples in the lecture: measles 2–3 years; rubella 6–9; influenza pandemics 7–10; plague pandemics about 10. • Secular = LONG-TERM trend: progressive increase or decrease over years/decades. • Trend analysis follows population disease patterns; natural history follows disease development and outcomes. ④ Cross-sectional vs longitudinal — تكررت بكثرة • Cross-sectional = SNAPSHOT: exposure and disease measured at one point in time. • Cross-sectional mainly measures PREVALENCE and cannot establish whether exposure preceded disease. • Cross-sectional: quick, relatively inexpensive, no follow-up attrition; cannot track natural history or seasonal changes with one survey. • Longitudinal = repeated observations over time: like a FILM. • Longitudinal can measure INCIDENCE, natural history, future outcomes and seasonal variation. نقدروا نكتشفوا الحالات الجديدة؛ لهذا incidence of skin disease among workers → longitudinal. • Longitudinal disadvantages: more time, harder organization and loss to follow-up—ATTRITION. ⑤ Case report and case series • Case report = detailed description of ONE patient. • Case series = several similar cases, without a comparison group. • Case series cannot calculate incidence/prevalence without a defined population denominator, or establish causation.

Concept of health: PQLI — Physical Quality of Life Index
Concept of health: PQLI — Physical Quality of Life Index

⑥ PQLI vs HDI — ما تخلطوش المؤشرات • PQLI = Infant Mortality Rate + Life expectancy at AGE ONE + Literacy. • PQLI: each indicator scores 0–100; the three scores are averaged. • HDI = Longevity + Education + Income. . ⑦ Determinants of health • Determinants = factors influencing health; indicators = measures describing health status. • Socioeconomic → income, education, occupation. Biological → heredity. Behavioral → lifestyle. • Environment → housing, water and psychosocial conditions. Health services → availability, accessibility, affordability and quality. ⑧ Health philosophy and UHC • Health is a human right, part of development, a social goal and investment, and central to quality of life. • UHC = access to needed quality healthcare without financial hardship—for everyone, everywhere. @OSOS_team

#Concept of Health — Key exam points ① Changing concepts of health — كيف توسّعت الفكرة؟ • Biomedical → Ecological → Psychosocial → Holistic. • Biomedical: health = absence of disease. • Ecological: dynamic equilibrium between man and his environment. احفظوا man–environment balance. • Psychosocial: social, psychological, cultural, economic and political factors influence health. • Holistic: multidimensional well-being of the whole person within the environment; all sectors affect health. ② WHO definition • Health = complete physical, mental and social well-being—not merely absence of disease or infirmity. • Original WHO dimensions: PHYSICAL + MENTAL + SOCIAL. Spiritual “Health is a dynamic state of complete physical, mental, spiritual and social well-being, and not merely the absence of disease or infirmity.” ③ Dimensions of health — فرّقوا بينهم • Physical → body function, fitness and Activities of Daily Living (ADLs). • Mental → thinking clearly and coherently; psychological balance. • Social → harmonious interaction and integration with society. • Spiritual → inner peace and harmony, connected with beliefs and practices. • Emotional → appropriate expression and control of emotions; self-esteem and confidence. • Vocational → work adapted to the person’s goals, capacity and limitations. • Other dimensions: philosophical, educational, cultural, environmental and behavioral. PRIMARY HEALTH is not a dimension. ④ Spectrum of health — تكررت أكثر من مرة • Positive health → Better health → Freedom from sickness → Unrecognized sickness → Mild sickness → Severe sickness → Death. • Transition is GRADUAL, with NO single cut-off point. • Health is DYNAMIC: maintained and renewed, NOT* attained once and for all. ⑤ Well-being — objective vs subjective • Standard of living = OBJECTIVE: education, employment, housing, clothing/lifestyle, recreation and material comforts. • Quality of life = SUBJECTIVE: happiness, satisfaction, freedoms, rights and privacy. Higher income does not necessarily mean better quality of life. • Today, quality of life is as important as years of life. • Level of living has nine components: health, food consumption, education, occupation, housing, social security, clothing, recreation and human rights. احفظوا القائمة كإطار بروحه؛ human rights موجودة هنا، لكن في مقارنة standard of living مع quality of life تتحطّ تحت quality of life.
