Real causal signals in epidemiology tend to be enormous — smoking raised lung cancer risk roughly twentyfold, which is why nobody could explain it away. When a headline reports a slight excess risk near golf courses or dry cleaners, the honest read is that something might be there and the study can't tell you. This standard cuts in every direction, against findings you like and findings you don't. Applying it selectively is how you end up believing whatever you already believed. @PeterMcCulloughMD .

Most sore throats, coughs, and sinus congestion in the first week are viral, and an antibiotic won't shorten them by a day. What it will do is disrupt your gut flora, risk a reaction, and add to resistance. The signals that actually warrant a call to your clinician are symptoms persisting past ten days, or improving and then clearly worsening — the pattern that suggests something bacterial has set in behind the virus. 1721: A Boston Doctor Settled a Furious Medical Argument by Publishing His Numbers Zabdiel Boylston inoculated roughly 280 people during the smallpox epidemic and about six died — near two percent, against roughly fourteen percent among those who caught the disease naturally. He didn't win the argument by authority or by shouting. He counted, published, and let anyone check. That's one of the earliest uses of outcome data to resolve a clinical dispute in America. Boston's physicians dismissed it — the source had no standing, and the idea was foreign. Mather's house was firebombed for promoting it. The practice worked. It's worth remembering that medicine's most important early advance in prevention arrived from outside the credentialed establishment, and the establishment's first instinct was to refuse it. Before modern plumbing, nearly every infant encountered poliovirus in the first months of life — while still carrying maternal antibodies. Exposure was near-universal and paralysis was rare. Clean water broke that cycle, pushing first exposure into later childhood and adulthood, when paralytic disease is far more likely. The epidemics of the 1940s and 50s were a byproduct of progress. It's the clearest example in medicine of an intervention producing a serious harm nobody anticipated, and it should make everyone humbler about second-order effects. After 1954, the case definition tightened to require residual paralysis at sixty days, and laboratory confirmation became standard. Reported counts fell for both reasons. None of this settles what the vaccine did or didn't accomplish. It does mean that raw before-and-after numbers, cited by anyone in this argument, are measuring two different things. Clean water, sewage systems, better nutrition, and less crowding cut mortality from measles, scarlet fever, typhoid, and tuberculosis long before any vaccine for them was available. Mortality and incidence are different measures, and conflating them distorts the history in both directions. The honest accounting gives sanitation and nutrition the enormous credit they earned — and it's also the reason public health should be investing in those foundations now, not only in the products that generate revenue. The Vaccination Acts of 1853 and 1867 made smallpox vaccination compulsory for infants, with repeated fines and imprisonment for parents who refused. In 1885, tens of thousands marched in Leicester against it. Not until 1898 did Parliament add a conscientious objection clause — the origin of the term as we use it today. The lesson isn't about smallpox. It's that the right to decline a medical procedure was never granted freely. It was fought for, lost, and fought for again.
