Onesimus described to Cotton Mather a procedure long practiced in West Africa: deliberate inoculation with material from a mild smallpox case. Boston's physicians dismissed it — the source had no standing, and the idea was foreign. Mather's house was firebombed for promoting it. The practice worked. It's worth remembering that medicine's most important early advance in prevention arrived from outside the credentialed establishment, and the establishment's first instinct was to refuse it.

| 3 | Polio Became an Epidemic Disease Because Sanitation Improved. Not Despite It.
Before modern plumbing, nearly every infant encountered poliovirus in the first months of life — while still carrying maternal antibodies. Exposure was near-universal and paralysis was rare. Clean water broke that cycle, pushing first exposure into later childhood and adulthood, when paralytic disease is far more likely. The epidemics of the 1940s and 50s were a byproduct of progress. It's the clearest example in medicine of an intervention producing a serious harm nobody anticipated, and it should make everyone humbler about second-order effects.

| 5 | The Definition of a Polio Case Changed in the Middle of the Vaccine Era
Before lab confirmation was routine, paralytic polio was a clinical diagnosis — and other enteroviruses producing similar presentations were counted alongside it. After 1954, the case definition tightened to require residual paralysis at sixty days, and laboratory confirmation became standard. Reported counts fell for both reasons. None of this settles what the vaccine did or didn't accomplish. It does mean that raw before-and-after numbers, cited by anyone in this argument, are measuring two different things.

| 6 | Death Rates From Infectious Disease Fell Dramatically Before Most Vaccines Existed. That Part Isn't Controversial.
Clean water, sewage systems, better nutrition, and less crowding cut mortality from measles, scarlet fever, typhoid, and tuberculosis long before any vaccine for them was available. Mortality and incidence are different measures, and conflating them distorts the history in both directions. The honest accounting gives sanitation and nutrition the enormous credit they earned — and it's also the reason public health should be investing in those foundations now, not only in the products that generate revenue.

| 7 | Britain Jailed Parents Over Vaccination. It Took Thirty Years of Resistance to Win an Exemption.
The Vaccination Acts of 1853 and 1867 made smallpox vaccination compulsory for infants, with repeated fines and imprisonment for parents who refused. In 1885, tens of thousands marched in Leicester against it. Not until 1898 did Parliament add a conscientious objection clause — the origin of the term as we use it today. The lesson isn't about smallpox. It's that the right to decline a medical procedure was never granted freely. It was fought for, lost, and fought for again.

| 8 | A French Firm Helped Build the BSL-4 Lab in Wuhan. Its Own Scientists Warned Against It.
The 2004 Franco-Chinese agreement to construct a maximum-containment facility at the Wuhan Institute of Virology drew objections from French defense and scientific officials at the time, concerned about dual-use risk and whether promised oversight would materialize. It didn't — the French scientists slated to work alongside Chinese researchers were largely never integrated. The safeguards that justified the project were abandoned, and the lab operated anyway. The warnings are in the record. Someone decided the partnership mattered more.

| 10 | When a Countermeasure Is Declared, You Lose the Right to Sue. Almost Entirely.
The PREP Act of 2005 grants near-total immunity to manufacturers, distributors, and administrators of a declared countermeasure — no design defect claims, no failure-to-warn claims, no negligence claims. The only exception is willful misconduct, a bar so high it has essentially never been cleared. The alternative is a federal compensation program that has denied the overwhelming majority of COVID claims filed. A book takes up the question of how vaccination became a matter of identity rather than evidence — where declining one product marks you as belonging to a camp, and where defending the entire category is treated as a credential. Neither posture is science. Vaccines differ enormously from one another in risk, benefit, and evidence quality, and they should be evaluated individually. The argument here is that we lost the ability to do that somewhere along the way, and getting it back is the actual work.

| 12 | "There's No Treatment" Was Never True. It Was a Default.
Monkeypox, bird flu, hantavirus, Ebola — each arrives with the same framing: nothing to do but wait for a vaccine. But supportive care changes outcomes in nearly all of these. Fluids and electrolytes. Antivirals where they exist. Early recognition. Ebola case fatality dropped substantially with aggressive supportive treatment alone, long before any vaccine existed. Declaring a disease untreatable isn't a clinical finding. It's a choice about where to point the money.
