Patients with ulcerative colitis are at increased risk for colorectal cancer, and the risk is proportionate to the duration and extent of disease. Screening colonoscopy should be offered to patients with long-standing ulcerative colitis beginning 8 years after diagnosis and every 1-3 years thereafter.

Patients with severe Crohn disease, especially those who have required intestinal surgery in the past, are at high risk for future complications and often need aggressive management with biologic and/or immunomodulator therapy. Smoking is strongly associated with increased severity and progression of Crohn disease and should be avoided in these patients

Crohn disease can involve any portion of the gastrointestinal tract from the mouth to the anus. Oral aphthous ulcers and perianal manifestations (eg, fissures, skin tags, fistulae) are common. Histologic findings include transmural inflammation and noncaseating granulomas.
