الكليشة المهمة للجهاز التنفسي بالاوسكي:

Type of Breathing:
*Harsh Vesicular Breathing
Means Normal (Inspiration More Than Expiration). *Broncho-Vesicular Breathing Means Obstructive Lung Diseases (Expiration More Than Inspiration) as in Asthma, COPD, Bronchiectesis.
💭No prolongation of the inspiratory or expiratory phases
💭No rhonci (if present we should mention the area , highest area , expiratory or both)
💭No crepitation (if present we should mention the area , fine or coarse)
💭No friction rub
💭No stridor (harsh inspiratory sound which is smiliar to rhonci sound but in the inspiratoey phase)
💭 DDx
What you say in ( mention the area ) :
1- left upper zone
2- right upper zone
3- left middle zone
4- right upper zone
5- left lower zone
6- right lower zone
اذا سمعتوا بكل الاماكن تكدرون تعبرون علي بالطريقة الي تعجبكم لكن اسهلها هي :

💥Vocal resonance
Causes of increased vocal resonance: consolidation and collapse
Causes of decreased vocal resonance: pleural effusion, pneumothorax, collapse with obstructed bronchus.
للي عدهم consolidation نسويلهم شي اسمه Whispering pectoriloquy نكله اهمس بصوت ناصي ونسمع بالسماعة فحنلكه الصوت عالي عبالك ديحجي بصوت عالي
وهمات نسوي شي اسمه Aegophony وهذه هنا نكوله كول eeeeee حنسمعها aaaaa
- Crackles it a deep breathing to represent the sudden opening of small airways
Coarse Crackles :
Good air entry all over the lung
Normal vesicular breathing all over the lung without prolongation of expiratory phase
Regarding added sounds there is biphasic coarse crackles in ( mention the area )
DDx :
1- T.B
2- absseces
3- resoulation phase of pneumonia
4- bronchiectasis
5- advance pulmonary odema
Fine Crackles :
Good air entry all over the lung
Normal vesicular breathing all over the lung without prolongation of expiratory phase
Regarding added sounds there is inspiratory fine crackles in ( mention the area )
DDx :
1- atypical pneumonia
2- interstital lung disease
3- pulmonary fibrosis
4- early phase of pneumonia
5- pulmonary odema
Wheezing :
Good air entry all over the lung
vesicular breathing all over the lung with prolongation of expiratory phase
Regarding added sounds there is expiratory wheezing in ( mention the area )
DDx if wheeze all over the lung :
1- asthma
2- chronic bronchitis ( COPD )
3- Bronchiolitis
4- acute bronchitis
5- cardiac asthma
DDx if wheeze in localized area :
1- tumor
2- forgien body
3- mucus inspiration
4- compression from outside
5- bronchiectasis
