الهيباين heparin الاسم العلمي اله هو Unfractionated heparin UFH (or high molecular weight heparin HMWH) اسمه هيبارين لان اول مصدر اله هو الكبد hepatic ١. الي متوفر عدنا بالطوارئ هو فيالة 25000U/5cc يعني 5000U/1cc 2. Mechanism of action: -- main mechanism is activation of anti-thrombin III 👉👉 degrade factor Xa 👉👉 prevent further clot formation فلهذا اذا شخص عنده anti thrombin III deficiency، ما راح يشتغل عليه الهيبارين مثل بعض حالات الـ nephrotic syndrome (لان راح ينطرح بالادرار). 3. Indications and dosage regimen: A. Non ST elevation ACS and STE ACS with alteplase 👉👉 60 U/kg as a IV bolus dose + 12 U/kg/hr as a maintenance dose. B. STE ACS with PCI 👉👉 70-100 IU/kg (max 10000) iv bolus dose. C. Pulmonary embolism 👉👉 80 U/kg (max 10000) IV bolus dose + 18 U/kg maintemance dose D. Prophylaxis against DVT 👉👉 5000 U S.C every 8 hours 4. Monitoring: بعد ٦ ساعات من اعطاء الهيبارين 👈👈 تسحب دم و تجيك الـ aPTT، التاركت هو 60-80 ثانيه اذا كان الـ aptt اقل من التاركت 👈👈 فنبدي نرفع الـ infusion rate بمقدار 2-4 يونت لكل كيلو بالساعه. يعني اذا كان ماشي ع 1000U بالساعه و هو وزنه 80 👈👈 نخلي الدوز 1240 (يعني 3U/kg) اذا كان ال aptt اكثر من التاركت، فنقلل ال infusion rate بمقدار 2-4 يونت لكل كيلو بالساعه. اذا كان الـ aptt اكثر من النورمل و المريض بدء ينزف بأماكن خطرة 👈👈 نوقف الهيبارين و ننطي protamine sulfate 👈👈 تنطي 1mg من البروتامين لكل 100 يونت من الهيبارين المأخوذة خلال اخر ساعتين يعتي اذا المريض كان ماشي على هيبارين 1000 وحدة بالساعه من الساعه الـ ٤ عصرا ، و صار عنده نزيف الساعه الـ ٢ صباحا يعني من الـ ١٢-٢ المريض ماخذ 2000 U 👈👈 يعني ننطي بروتامين 20mg. البروتامين يجي ع شكل 50mg/5cc 👈👈 نضيف الجرعه المحسوبه ب 100cc NS و نمشيه خلال 5-10 دقائق، و كلش خليك حذر من ال Hypersensitivity reaction البروتامين الكلام يطووووول جدا حول الهيبارين و البروتامين لكن هذا المهم #emergency #pharma @AASmedicalbot
