📌 ICP: Timing of Birth & Intrapartum Care Once you understand the diagnosis and follow-up of ICP, the next question is: When should she deliver, and does she need continuous CTG? 🟢 Mild ICP | Bile acids 19–39 μmol/L • Stillbirth risk is similar to the background population. • Consider planned birth by 40 weeks, or ongoing antenatal care according to national guidance. 🟡 Moderate ICP | Bile acids 40–99 μmol/L • Stillbirth risk remains similar to the background risk until 38–39 weeks. • Consider planned birth at 38–39 weeks. • With peak bile acids <100 μmol/L, there is insufficient evidence for or against routine continuous electronic fetal monitoring solely because of ICP. Individualise according to other risk factors. 🔴 Severe ICP | Bile acids ≥100 μmol/L • Stillbirth risk is higher than the background risk. • Consider planned birth at 35–36 weeks. • In labour → Offer continuous electronic fetal monitoring (CEFM). ⚠️ MRCOG Exam Point: Women with moderate or severe ICP are more likely to have meconium-stained liquor, which may influence the need for continuous fetal monitoring during labour. 🧠 Easy numbers to remember: 19–39 → 40 weeks 40–99 → 38–39 weeks ≥100 → 35–36 weeks + CEFM Remember: gestational diabetes, pre-eclampsia, multifetal pregnancy and other obstetric or medical risk factors may increase the risk and influence both the timing of birth and intrapartum monitoring. 📚 Know the bile acid level → assess the risk → choose the right timing of birth.
