Hashimoto's encephalopathywhich is extremely rare. You should suspect this as elevated TSH is present with comatse patient however there may be no clinical evidence of thyroid dysfunction. It can result in altered mental state, myoclonus and ataxia. It's a steroid responsive encephalopathy. #Notes
Presence of wide QRS or bundle branch block (BBB) is associated with a worse prognosis.
Evidence from one meta-analysis showed that BBB of any type (right or left) was associated with a modest increase in the risk of all cause mortality in patients with acute heart failure.
