السكر العالي مش طوارئ العيان دا ممكن جدل يبقي abdominal pain و معاه inferior MI و أخر حاجه تهمك ف ال dka انك تنزل السكر أولا عالج ال dehydration خصوصا السن الكبير و شوف ال electrolyte diaturbance & metabolic acidosis و أخر حاجه متشلش ذنب عيان علشان مكسوف أو متعرفش أو انك لسه غير مؤهل د. شادي يوسف

أولا: مع أي حالة ارتفاع نسبة السكر لازم تتوقع اسيتون في البول خصوصا ان معاه abdominal pain & vomiting ثانيا: جرعة الأنسولين دي كبيرة جدا جدا دي تقريبا أكتر من سبع أضعاف الجرعة اللي المفروض ياخدها يعني في الحالة دي لو مفيش أسيتون في البول🙆🙆🙆 0.1U per kg repeated every one hour طبعا مع متابعة نسبة السكر في الدم كل ساعة ومحاليل وريدية صديقي طبيب الامتياز: والله ما  عيب لو قلنا مش عارفين،   انت محتاج تقول إنك مش عارف علشان تتعلم وتبقي شاطر .. كنت حول الحالة حتى لو مكنش فيه كيتون في البول واخلي مسؤوليتك... لذلك نصيحة مني بلاش شغل برايفت في الامتياز إلا لو اتعلمت واتدربت صح . الست اللي سكرها ٥٢٥ وفي وجع و ترجيع كان لازم تفكر في حاجتين: DKA and myocardial infarction والاتنين أخطر من بعض ومحتاجين تحويل لمستشفي ..😃

زملائنا الامتياز في راوند الباطنة: مهم جدا تركز على إزاي تشخص الحالة مش إنك تعالجها العلاج بشكل عام سهل لكن الباطنة هي التشخيص . حضور مرور الأساتذة والاستاف مهم لأبعد حد لأنه بيخلص لك عصارة الكتب والخبرة في كلمات حاول تستخرج منهم إزاي الحالة اتشخصت بأقل فحوصات ودي فكرة هتلاقيها في أسئلة ال MRCP تلاقيه كاتب لك معطيات قليلة جدا تشخص منها الحالة . حاول تتعلم تكتب فحص إيه ؟! وليه ؟! وإمتى ؟! ومعنى كل فحص منهم للحالة إيه ؟! وإزاي تترجم الفحوصات وتربطها بالحالة والأعراض . ركز إزاي تلقط الحالة الصعبة من وسط ألف حالة ركز على شكل المريض ووضعه وشكوته تتعلم إزاي تقرأ تحاليل وأشعة مش لازم بالتفصيل لكن تعرف الحاجات الكبيرة فيها . ارشق للنايب واسمع كلامه علشان هيعلمك واللي عايز يتعلم هيتعلم حتى لو بعتك ديرتي في بنك الدم مثلا استغله للتعليم ومعلومة زيادة شوف كيس الدم شكله ايه وشوف موجود فين وإزاي وليه وايه البيانات المكتوبة عليه واسأل نايب بنك الدم على سؤال سريع عايز تعرفه أو فني بنك الدم او تمريض البنك وهكذا المهم حاول تعود نفسك تطلع من اي مشوار داخل المستشفى بمعلومة . مهم جدا تعرف تشخص الحالات المشهورة وإيه العلاج المهم لها حالات الكبد حالات الغدد حالات الدم وهكذا . وأي معلومة تعرفها اكتبها في نوتس معاك بورقة وقلم افضل من الموبايل ولو مش حابب حد يشوفك اكتبها على تليجرام او نوتس الموبايل او ابعتها لنفسك صوتية على الواتس وهكذا واكتبها باللغة الدارجة لك بأسلوب سهل جدا تفهمه بعد ألف سنة . لازم تتعلم شغل التمريض والعمال ومش ضروري تعرف حد انك اتعلمت منه . شوف ايه الحالة المهمة في القسم والحالة العادية وإزاي نحدد خطورة الحالة وعلى أي أساس وإزاي تتابعها وشوف النايب بيسأل على ايه في الهيستوري وليه ؟! ولو عندك أسئلة كتير بلاش تسألها في وقت واحد ولكن سجلها واسأل على أيام علشان محدش يملّ من كثرة الأسئلة . لو حالة رايحة سونار روح معاها لو تعرف وشوف نايب الأشعة بيعمل ايه ومرة مع التانية هتتعلم حاجات ولو بسيطة ولما النواب تعرف شكلك كتير هتبدأ تعلمك . خليك هادي مع أي حد وبلاش تخلق عداوة حتى لو اللي قدامك منحط اخلاقيا وادفع بالتي هي أحسن السيئة كلها فترة وتعدي ومحدش له عندك حاجة خليك هادي مهما كان وارشق على أد ما تقدر في حالات الطوارئ والصعبة علشان دي اللي هتخليك دكتور شاطر عن غيرك وإوعى تهرب من حالة ادخل البحر وهتتعلم العوم في يوم من الأيام وهتبقى بحار د.أحمد العسكري

بمناسبه بدايه الإمتياز والعمل في المستشفى بشكل عام : أول رمضان لي سيبقى خارج صومعتي ، هكذا أطلق أهلي هذا الاسم على غرفتي في رمضان للحالة الخاصة التى تكون فيها وندرة خروجي منها ..! أول رمضان وأنا لا أعلم سأعتكف أم لا ، أول رمضان وأنا لا أدري خلف من سأصلي التراويح ، وأي مسجد سأختم فيه ، أول رمضان بلا مخططات على أرض الواقع.. ! ولكن لعل الله فتح لي أبوابًا للأجور جديدة .. -تخفيف ألم ، وإماطته عن جسد مسلم ، فإماطة الأذى عن الطريق صدقة ، فما بالها عن المسلم نفسه ! -نية زيارة المريض اليومية ، وهذا فيه من الفضل ما فيه ولكن ما يقشعر له بدني أن الله قال عن نفسه ( أما علمتَ أنَّ عبدي فلانًا مرض فلم تعده، ولو عدتَه لوجدتني عنده)! -دعوة مضطر . -تفريج كروبات . - هداية حائر في طرقات المستشفى. -التهوين و التيسير على الناس . - ذكر لله عند الانشغال، غير ذكر الله في الفراغ التام . - دعوة لله ، وتذكير به . - إعانه لزميلاتي على الطاعة . - لعل تلاوتي القرءان هناك ، تذكر من كان غافل عن ورده . - لعل الله يرزقنا أجر افطار صائمين . لو تأمل كل منشغل ، سيجد أبوابًا كثيرة يستغل بها نفحات دهره ، فسبحان الواسع الذي وسع لنا سبل رضاه . -منقول

◽️تم تحويل حالة من عمران لعمل bone marrow aspiration بسبب ان الحالة suspect leukemia وكتب بالملف كذا وعلى اساس تحولت للكويت لقسم الleukemia ✨ الحالة suspect leukemia لكن هناك ماهو confirm ويستدعي managment أسرع ايضا برمج اي دكتور بيمسكها على هذا التشخيص الحالة عندها fever ,cough, dyspnea two attacks of epistaxis mild amount abd.distension بالفحوصات الملفت ان الWBC 60 الف واكثر صحيح في حاجات تمشي مع الleukemia (وقد تكون) لكن مالم يتم الانتباه له من الهستوري ان الcough paroxysmal in character followed by vomiting + there is history of contact with pt complain of cough ايضاً بالفحوصات ارتفاع الWBC لهذه الدرجه (اكثر من 50 الف يسمى Leukmoid reaction)وهذه النقطه ربما اكثر شي تم التحويل لسببها ولم يتم الانتباه ايضاً ان D. D of leukmoid reaction .........whooping cough, sepsis, T. B, sacrodosis and the last D. D malignancy وماتم تاكيده ان الحالة whooping و كان المفترض كاولوية التعامل مع هذه الحاله as whooping cough....and start managment قبل التحويل وبعدها ممكن ينعمل لها BMA للتاكد #Clinical_Practice

السؤال الذي وصلنا من اغلب الزملاء كيف دوام الامتياز أولا الامتياز مرحله انتقاليه بين حياة الطالب والتحول الى طبيب ممارس للمهنة وفيها يكتسب الطبيب مهارات متعددة من مختلف الاقسام التي يمر بها ويتم التعامل معه كطبيب وليس طالب وفي عدد من المتطلبات اللي لازم يلتزم بها طبيب الامتياز ونختصرها بالتالي : 1. الالتزام بالحضور والبصمة حسب توزيع الطالب واهم شيء هي البصمة يعني ضروري مش أقل من 30 يوم يطلع لك بالبصمة 2 . حسن السلوك والمظهر اللائق بالطبيب 3 . احترام المشرفين والسنيور مش عارف جمعها الأهم الإلتزام بنظام المستشفى لأنها تختلف من مستشفى لاخر 4 . تجهيز البطائق والتصاريح لاجل الواحد ما يحنب ويتعرقل 5. الحرص على الاستفادة من كل الكادر الطبي من السنيور أو من التمريض لأنهم أكثر من تقدر تستفيد منهم في مرحلة الامتياز وخاصة الطوارى 6. الايام العلمية وما إدراك ما الايام العلمية الكل يحضرها لأنها مهمة واليوم حقه التعويض باسبوع في الطوارى 7. تجهيز اللوج بوك لكل قسم حسب اللي نزلوها من العمادة ومتوفره بمكتبة 48 مع الداتا ما عدا حق النسائية تاخذوا حق جامعة صنعاء من مكتبة التاج ثانيا : ملاحظات على التقسيم بمختلف الاقسام أولا : قسم النساء والتوليد في الجمهوري والسبعين والكويت الدوام بيكون للطالبات لكن الطلاب معاهم حضور يومي للميتنق وضروري التوقيع يومياً بينما في الثورة مسموح تدخل كل تفرعات القسم من مرور ومناوبات عمليات طوارى ..الخ ثانيا: الطوارى وطب المجتمع في الجمهوري دوام اربعه اسابيع مناوبات الثاني يوم يعني كل أسبوع معك 3 مناوبات وجمعة كل اسبوعين في الثورة نفس الشيء بس المناوبات تختلف يعني تدخل الرابعة عصراً تخرج اليوم الثاني الثامنة صباحًا بالنسبة لطب المجتمع هو أسبوع بضلاع همدان أو في الملصي القروب الفرعي ينقسم الى 2 قروباات بالدوام يعني لو العدد عشرين طالب بالقروب بنقسمهم الى 2 قروباات كل قروب من 10 طلاب ويتم التناوب على الدوام يوم بيوم ثالثا : الأطفال الكويت بيتم التقسيم لكل قروب الى 3  قروبات مصغره ويتم توزيع القروبات بين مرور بالقسم أو٥ الطوارى أو العيادات أو اللوكيميا بقية المستشفيات نفس التقسيم مابش فقط لوكيميا المناوبات يوم بالاسبوع وجمعه واحده بالشهر الدوام من السبت الى الاربعاء عدا المناوبين رابعا : الجراحة في الكويت تقسم الى ثلاث وحدات A B C وكل واحدة تداوم مع اليونت الخاصة بهم وبيكون معاكم عيادات عمليات رواند بالقسم المناوبات يوم بالاسبوع فقط واهم شيء بالدوام المناوبات بقية المستشفيات نفس التقسيم ويختلف بنظام المستشفى واكثر شيء هو المناوبات لأن السنيور ممكن ما يقيمك إذا ما كنت تحضر مناوبات خامسا: الباطنية الجمهوري أو الكويت بتتوزع حسب ما يشوفه السنيور ويمكن تتوزع باكثر من تفرع باطنيه ممكن تدخل عامه أو قلب والمناوبات يوم بالاسبوعف وجمعة كل اسبوعين بالثورة بتتوزع بمكان واحد طوال الشهرين المناوبات نفس الخبر سادسا: الماينور حسب ما تتوزع أنا توزعت الكويت جلدية وENT اسبوعين لكل واحدة وكان الدوام بالعيادات توزعنا بالجمهوري اشعة كان الدوام الثاني يوم ومافيش مناوبات توزعنا يورو بالثورة اسبوعين والدوام من السبت ألى الخميس ومناوبات من عصر الى التاسعة مساء أخيراً: البصمة بصمة الدخول من الثامنة صباحًا والخروج من 11:30 أو 12:00 مش أقل ضروري تقول لك البصمة شكراً واشارة الصح وشوف أحيان يطلع صح والذاكرة ممتلئة فالواحد يركز ويشوف بالبصمة وش يطلع طارت علينا اسابيع من هذا الخبر ورجعنا نعوض بصمة المناوبات وقت اضافي داخل ومعك وقت اضافي خارج نهاية المناوبات وموفقين أن شاء الله

80. Consider nonaccidental trauma (child abuse) in children when the injury pattern is incompatible with the developmental age, such as long bone fractures in a nonambulatory child. 81. Use a tourniquet on extremities and digits to control hemorrhage; visually examine and properly repair the wound. 82. Disasters and multiple casualty incidents (MCIs) occur whenever the needs of an incident exceed the resources available to respond to it. 83. Communication is the most important component to ensure an efficient and effective disaster response. 84. The “ideal” terrorist weapon is cheap, easy to manufacture, easy to disseminate, and will produce large numbers of casualties (e.g., an improvised or conventional explosive device). 85. A chemical agent attack will generally be recognized by having a large number of casualties with similar symptoms over a short time in a relatively small geographic area. 86. Percutaneous coronary intervention (PCI) is preferable to thrombolytics for emergency management of ST-segment elevation myocardial infarction (STEMI) unless there is an anticipated delay of more than 90 to 120 minutes to balloon inflation. 87. Documenting adherence to evidence-based guidelines is helpful in defending againsta malpractice claim. 88. Suspect ectopic pregnancy when there is no evidence of intrauterine pregnancy (IUP) by transvaginal ultrasound and the quantitative human chorionic gonadotropin (HCG) concentration is greater than 2000 IU/L. 89. Patients who have persistent hemodynamic instability or peritonitis after abdominal trauma require emergent laparotomy. 90. Never restrain a patient in the prone position; restrain the patient on his or her side tominimize risks of aspiration and sudden death. 91. In the event of a biologic agent attack, all symptomatic patients should be considered to be infectious until a definitive diagnosis of a non transmissible agent is confirmed. 92. Consider domestic violence in women with depression, suicidal ideations, chronic pain, psychosomatic complaints, or multiple ED visits. 93. Ingestion of a disc or button battery mandates an emergent GI or surgery consultation for endoscopic or surgical removal. 94. As little as 2 weeks of chronic steroid use (prednisone 20 mg/day) willcause adrenal suppression, making a patient more prone to adrenalcrisis. 95. Consider toxic shock syndrome in any patient with a rapidly progressive shock syndrome and diffuse erythematous rash, and ensure there is no removable infected source of endotoxin production. 96. A CT scan for appendicitis is negative only if the entire appendix has been visualized and is normal. 97. Consider cyanide poisoning when the patient has inhaled smoke from burning furniture fabric (e.g., wool, silk, or poly urethanes). 98. Obtain a CT scan of the head on any patient taking warfarin (Coumadin) with even a minor head trauma. 99. Emergency medical services (EMS) has proven benefit for treatment of cardiac arrest, respiratory distress, and traumatic injury. 100. Spinal epidural abscess should be suspected as the cause of back pain in immunocompromised patients and IV drug users who have localized spinal tenderness and fever.

56. Consider testicular torsion in any male with lower abdominal pain. 57. The antidote for acetaminophen overdose is N-acetyl cysteine. It is most effective when administered within 10 hours, regardless of whether it is given orally orintravenously. 58. Obtain a carbon monoxide (CO) level, and treat any patient who has inhaled smoke in an enclosed space with nonrebreather, high-flow, mask oxygen. 59. The classic triad of opioid poisoning is central nervous system (CNS) depression, respiratory depression, and miosis. 60. Because specific amounts of drugs ingested are often difficult to determine in smallchildren, prolonged observation is often required to rule out a potentially toxic ingestion. 61. Neither pregnancy nor tubal ligation excludes a diagnosis of pelvic inflammatory disease. 62. Victims of sexual assault must be told of the options they have as to reporting and evidence collection. They can report to law enforcement and have evidencecollected; they may decline reporting to law enforcement and have evidencecollection; or they can decline to report and decline evidence collection but chooseonly treatment. 63. A pregnant woman with hypertension and seizures should be treated with IV magnesium sulfate and consideration of emergent delivery of the fetus. 64. With few exceptions, procedures performed in the ED can be done with fewer complications and greater success using ultrasound guidance. 65. All trauma patients should be completely undressed and examined, front and back. 66. Use clinical decision rules such as the National Emergency X-Radiography Utilization Study (NEXUS) or the Canadian C-spine rule to avoid overuse of cervical spineradiographs. 67. The use of steroids in blunt spinal cord trauma is not standard of care. 68. In patients with traumatic brain injury, maintain cerebral perfusion and oxygenation, and avoid hypotension. 69. Preservation of vision in a chemical burn is directly related to the length of time from the exposure to the initiation of irrigation; do not wait for the patient to arrive at the ED to begin eye irrigation. 70. Diplopia on upward gaze is the hallmark of a blow-out fracture of the orbital floor. 71. In patients with blunt or penetrating neck trauma, perform endotracheal intubation early, before airway distortion occurs. 72. In a patient with chest trauma, immediate threats to life that must be identified and treated in the primary survey include airway obstruction, tensionpneumothorax, open pneumothorax, flail chest, massive hemothorax, andcardiac tamponade. 73. A contaminated wound is one with a high degree of bacterial inoculum at the time of injury, and is not synonymous with a dirty wound. 74. A single negative focused assessment with sonography for trauma (FAST) does not reliably exclude significant intraperitoneal injury. 75. In patients with pelvic fractures, mechanical stabilization of the pelvis, reversal ofshock, and correction of coagulopathy are critical early steps to avoidexsanguination. 76. Children manifest shock later than adults with the same percentage of blood loss, yet decompensate more quickly once this critical volume is lost. 77. Because of the laxity of children’s ligamentous structures, cervical fractures, rib fractures, and aortic injuries are less common, but cervical ligamentous injuries and pulmonary contusions are more common. 78. Scaphoid fractures can escape radiographic detection in the acute setting. Patientswith tenderness to palpation of the anatomic snuffbox should be treated with a thumb spica splint and repeat evaluation in 1 to 2 weeks under the assumption that they may have a fracture. 79. Any laceration over the dorsal metacarpophalangeal (MCP) joint is suspicious for afight bite. Fight bites require meticulous exploration and wound care in the operating room. If the wound penetrates the extensor hood, thorough joint washout and IV antibiotics are required.

29. An unstable patient with any tachydysrhythmia, regardless of the mechanism,requires electrocardioversion. 30. When trying to decide whether the rhythm your patient is in is ventricular tachycardia (VT) or supraventricular tachycardia (SVT) with aberrancy, assume VT and treat accordingly. 31. Calcium channel blockers should not be used to treat wide-complex tachycardias. 32. Consider ruptured abdominal aortic aneurysm in any patient with abdominal pain, pulsatile mass, and hypotension. 33. Consider HIV/AIDS in patients at risk and who have an illness or infection, particularly those with opportunistic infections or extreme presentations of common diseases. 34. Myocarditis should be considered in patients with significant tachycardia that cannot otherwise be explained, or in any patient with the combination of viral symptoms and evidence of cardiac disease. 35. Epigastric pain may be caused by myocardial ischemia, so an ECG should be obtained in adult patients with epigastric discomfort, visceral-type pain, or cardiac risk factors. 36. Consider mesenteric ischemia in any patient with atrial fibrillation and abdominal pain. 37. An external pacemaker can be used if a permanent pacemaker malfunctions. 38. Indications for emergency dialysis include acute pulmonary edema, life-threatening hyperkalemia, and life-threatening intoxication or overdose with agents normally excreted by the kidneys. 39. The ECG changes seen as potassium rises are a tall, peaked T wave; loss of the P wave; and widening of the QRS complex. 40. In the emergent treatment of hyperkalemia, glucose and insulin supplemented by aninhaled β agonist is the most effective method to drive potassium into the cell and acutely lower serum potassium. In addition, IV sodium bicarbonate and calcium chloride are indicated in patients with metabolic acidosis and a wide-complex cardiacrhythm. 41. Seizures, coma, and acute neurologic findings in a previously normal patient are the only indications to give hypertonic saline in patients with profound hyponatremia. 42. Anion gap metabolic acidoses are common and life threatening; the four major causes are lactic acid, ketones, renal failure, and toxic ingestions. 43. Always measure the serum glucose in patients who are agitated, violent, diaphoretic, or comatose, to rule out hypoglycemia as an easily treatable cause of these findings. 44. It is unnecessary to gradually empty the bladder when treating an episode of acute urinary retention. 45. TOP. 100. SECRETS IN. EMERGENCY 1. Always consider the most serious possible cause of the patient’s presenting signs and symptoms. 2. Treat ventricular fibrillation (VF) with immediate defibrillation if the arrest is witnessed;treat with cardiopulmonary resuscitation (CPR) and then defibrillation if the arrest isunwitnessed. 3. If the arrest is result of pulseless electrical activity (PEA), remember its common reversible causes (i.e., hypovolemia, hypoxia, cardiac tamponade, tensionpneumothorax, hypothermia, massive pulmonary embolism, drug toxicity, electrolytedisturbances, acidosis, and myocardial infarction) and treat them appropriately. 4. Preoxygenation with 100% fraction of inspired oxygen (FiO2) for 5 minutes or 8 vital capacity breaths, as well as passive apneic oxygenation, are critical steps to prevent oxygen desaturation during rapid-sequence intubation (RSI). 5. Never paralyze a patient unless you are certain that his or her airway can be ventilated with a bag-valve mask or an extraglottic rescue device. 6. Serum lactate is a commonly used marker to assess the extent of systemic hypoperfusion and the response to resuscitation. 7. The primary resuscitation goals in patients suffering from shock are to maximize oxygenation, establish adequate ventilation, improve hemodynamic distribution, and treat the underlying cause. 8. Nitrates are first-line pharmacotherapy for the treatment of CHF, and diuretics should be reserved for those patients with fluid overload.
