السلام عليكم ورحمة الله وبركاته،
إخواني وأخواتي الأعزاء،
أتقدم إليكم اليوم بطلب إعفائي وتنازلي عن منصبي كنائب لرئيس اللجنة العلمية وعضويتي فيها. لقد كان العمل معكم ولأجل خدمتكم من أحب الأعمال إلى قلبي، وكنت أتحمل كل ضغوط العمل بكل رحابة صدر وضمير مرتاح، واضعاً مصلحة دفعتي وزملائي فوق كل اعتبار.
لكن للأسف، وصلت إلى مرحلة لم يعد الاستمرار فيها ممكناً بسبب الصعوبات والمشاكل مع اللجنة المركزية، التي باتت تفتقر للحرص على مصلحة الزملاء أو حتى احترام جهودهم او ارآئهم، ولم يتعاملوا معنا حتى بكلمة طيبة بالرغم من الزمالة التي بيننا.

1. LARYNGEAL CARCINOMA
A. SUPRAGLOTTIC CARCINOMA
≈ 15%
Early symptoms:
• Throat pain
• Dysphagia
• Referred otalgia
• Cervical lymphadenopathy may occur relatively early
Late symptoms:
• Dyspnea
• Weight loss
• Halitosis
• Respiratory obstruction
Memory:
• SUPRAGLOTTIC = SWALLOW + EAR
• Dysphagia + Referred otalgia

B. GLOTTIC CARCINOMA
≈ 80%
Early symptom:
• Persistent hoarseness of voice
Late symptoms:
• Dyspnea
• Stridor / airway obstruction
Memory:
• GLOTTIC = VOICE
• Persistent hoarseness is the classic early presentation

C. SUBGLOTTIC CARCINOMA
≈ 5% — Rare
Early symptoms:
• Stridor
• Dyspnea
Late symptom:
• Hoarseness

• SUBGLOTTIC = AIRWAY

2. VOCAL FOLD PARALYSIS
Unilateral Vocal Fold Paralysis
Usually, immediate treatment may not be required if:
• Airway is adequate
• Voice is acceptable
• Swallowing is safe
• Recovery is possible
Management:
• Observation and follow-up
• Voice therapy
• Injection laryngoplasty
• Medialization procedures when persistent or symptomatic
Main problems:
• Hoarseness / dysphonia
• Aspiration may occur

3. BILATERAL VOCAL FOLD PARALYSIS
Bilateral Abductor Paralysis
• Vocal folds are usually in a median or paramedian position
Mechanism:
• Posterior cricoarytenoid (PCA) paralysis
↓
• Failure of vocal fold abduction
↓
• Vocal folds remain near the midline
↓
• Narrow glottic airway
↓
• Dyspnea + inspiratory stridor
Main danger:
• Airway obstruction
Treatment:
• Tracheostomy — if significant airway obstruction
• Cordectomy / posterior cordotomy — to enlarge the glottic airway
• Vocal fold lateralization — to improve the airway
• Reinnervation / nerve-muscle implantation
Example:
• Sternohyoid muscle → Posterior cricoarytenoid muscle

4. VOCAL FOLD FIXATION IN LARYNGEAL CARCINOMA
Vocal fold fixation can result from:
1. Deep infiltration of the tumor
• Invasion of deep tissues / intrinsic laryngeal muscles
2. Cricoarytenoid joint invasion
• Mechanical restriction of arytenoid movement
3. Recurrent laryngeal nerve destruction
• Paralysis of intrinsic laryngeal muscles
High-Yield Point:
• Vocal fold fixation ≠ always paralysis
• The vocal fold may be mechanically fixed by cricoarytenoid joint invasion, even when the nerve is intact

6. DEEP NECK INFECTION — CLINICAL CASE
Case:
• A 30-year-old male presents with trismus and fever
• Swelling pushes the tonsil medially
• Swelling spreads laterally posterior to the middle of the sternocleidomastoid
• History of third molar extraction due to dental caries
Diagnosis:
• A. Retropharyngeal abscess
• B. Ludwig's angina
• C. Submental abscess
• D. Parapharyngeal abscess
Answer:
• D — Parapharyngeal abscess
Important clues:
• Dental infection / extraction
↓
• Deep neck space infection
• Trismus
• Medial displacement of the tonsil
• Lateral neck swelling
These findings strongly suggest:
• Parapharyngeal abscess

The most common cause Chronic liver cirrhosis.
other causes chronic renal failure, chronic heart failure, diabetes. aging.
Investigations, LFT, RFT, ECG..
Note other symptomes as jaundice, do LFT
Management: Treat underlying cause.

Diagnosis: Half-and-half nails (Lindsay nails) Diagnostic justification: A sharply demarcated proximal pale half with a distal red-brown band is the classic hallmark. As illustrated, multiple fingernails show symmetric proximal pallor and distal brown-red discoloration. The accompanying close-up confirms the characteristic transverse two-tone pattern. Dermoscopy demonstrates the corresponding sharply contrasting proximal and distal nail coloration.
Management: Evaluate and treat the underlying chronic kidney disease; .supportive nail care
