Maternal & perinatal mortality: -أهمية هذا الموضوع، في الأوسكي أكثر مِن كونه في امتحان الـPaper، من سيمسترين فاتُو، جابوه لأوّل مرَّة في أحد أيام امتحان الأوسكي. -كامتحان Paper فممكن سؤال واحد بالكثير. ----------------------------- ❇️ Maternal mortality: •Definitions: *Maternal death: -Is death of woman while pregnant or within 42 days of termination of pregnancy, due to any cause related (as abruptio placenta) or aggregated by pregnancy (as renal failure aggregate during pregnancy), or it's management., but not due to an incidental cause. - هو وفاة المرأة خلال فترة الحمل، أو خلال 42 يوم بعد انتهاء الحمل سواء كان بولادة أو اجهاض..، بسبب له علاقة بالحمل، أو ازاد سوءً خلال الحمل، أو يكون ناتج عن سوء معاملة مع الحالة المرضية للمرأة الحامل.. وليس له علاقة بأي حوادث خارجية، مثل حادث السيارة أو غيره. *Direct death: -Death resulting from any complications of pregnancy (as Antepartum Hg, post partum Hg, pre elampsia,... ect) *Indirect death: -It is death from previously existing disease or disease developed during pregnancy, not due to direct obstetric cases. سبب الوفاة هو إنّ المرأة الحامل كان عندها مرض أو صار معاها خلال الحمل، وليس له علاقة بأي سبب من مشاكل الحمل مثل: Abruptio placenta, placenta previa.. ect) *Late death: - Death occurring between 42 days and one year (after delivery or miscarriage). *Co incidental death: -death unrelated to pregnancy (as car accident). ---------------—------------ ☆Maternal mortality rate: According to WHO is death per 100,000 women. Important ‼️) عدد الوفيات من النساء الحوامل، من كل 100,000 سيدة حامل. ---------------—------------ ☆Maternal mortality ratio: -It is number of death per 100,000 birth. عدد الوفيات التي تحدث من كل 100,000 ولادة. ---------------—------------ •Incidence: According due to WHO > 600,000 maternal death per year. أكثر من 600,000 وفاة كل سنة. Highest in developed countries. Mortality rate at TMC 76 per 100,000. معدل الوفايات في مستشفى طرابلس الطبي، 76 حالة وفاة من أصل 100,000 ولادة. ---------------—------------ •Aetiology: 1) Thromo-emolism→leading cause in developed countries. 2) Hemorrhage→leading cause of in developing countries. (as APH, PPH.. Most commonly is Post partum hemorrhage). 3) Hypertension (Pre-elampsia, gestational hypertension, or chronic hypertension) b/c leading to: -Elampsia. -Intra cranial hemorrhage. -Cerebral infarction. -Multi organ failure (Rena l& hepatic) -DIC (Disseminated intravascular coagulation) 4) Amniotic fluid embolism →which is rare and unavoidable. نادر إنّ يصير، وما تقدرش تمنعه إنَّ يصير. 5) Sepsis (Chorioamnionitis, UTI or Post partum endometritis) 6) Other cause as: -Anaesthesia. -Acute fatty liver of pregnancy. -Psychiatric disease. 7) Indirect causes as cardiac disease. 8) Causes of death in early pregnancy (1st trimester): -Ectopic pregnancy. -Septic abortion. -Ovarian hyper stimulation syndrome. -Hyperemesis gravidarum. 🔴 To prevent mortality because Early pregnancy: *Should always think in Ectopic pregnancy. ‼️ *Heparin prophylaxis in case Ovarian hyper stimulation syndrome and Hyperemesis gravidarum and thiamine supplements.‼️
