محطة دكتور اشرف : مخلي صور بالايباد ويريد اسم fracture ،وكلما تجاوبه يطلعلك صورة ثانية ،اني ذنيّ جابهن الي Humeral shaft fracture Posterior shoulder dislocation Anterior shoulder dislocation Olecranon fracture Supracondylar fracture Captilum fracture وزاد وقت وسألني بعد على ثنين -نسيتهن 😞- محطة دكتور علي عباس :صور عن spinal imaging ،وطلب Medication اللي استخدمهم في back pain محطة دكتور جميل : 1-Femoral neck fracture وشلون تسوي ّ Management الها وطلب classification الها 2- صورة عبارة عن OA جانت joint space narrowing Osteophyte محطة دكتور ريماتو : مخلي صورة Dexa scan فراد الاسم ،والمن يستخدم والT,Z على ايّ شيء تدل محطة دكتور صحة : Patient with RTA ومخلي X-ray جانت humeral shaft شلون تسوي Management محطة دكتور صحة : Faber test المن نستخدمه وبيا disease يكون positive

سشن البارحـة🤝.

Emergency ( ATLAS ) سشن د. اشرف - Indication for atlas ? A: multiple injured patient. 1. Injury severity score اعلى او يساوي 15 نقسم ال body الى 9 regions وننطي score من 1 الى 6 ( الاسوء ) لكل region ( head, neck, thorax, upper limbs, lower limbs, pelvis,… ) وناخذ مجموع مربعات اعلى 3 scores مثلا: ال chest طلع 2 والابر 2 والابدومن 3 والبقية على 1 فتصير 4,4,9 فحاصل جمعهن 17 اذن يحتاج ATLAS ،، بس هذا صعب نسويه بالطوارئ.. 2. Involvement of more than 1 system/ haemodynamically unstable Needs : ATLAS * haemodynamic unstable ( tachycardia, tachypnia, hypotensive, low SPO2 اقل من 80 ) ex: In femur fracture ( lost 2 litres from haematoma ) ATLAS: ABC = primary survey A: airwat ( tube of breathing )  + cervical spine fixation B: breathing ( function of respiration الماطور ) C: circulation + Control of major bleeding D: disability الكسور + glascow coma scale E: exposure and environmental control ( ex: hypothermia ) ونطبق معا في ان واحد حسب الحاجة مو شرط بالسره،، + resiscitation ( compensated against hemodynamic instability ) + 3 main emergency x rays: - chest ( pneumothorax,… ) managed by chest tube - pelvis ( open book fracture ) الحل اقلل الحجم * pelvic binder اذا مافاد ➡️ damage control S.X حتى احصل tamponade يعني pressure from inside stop bleeding * pelvic binders الاصلي الي ما راح نشوفه مثل الجلابتين نثبته ع greater trochanter احنه بداله نستخدم مبدأ toweling اي غطاء طويل او ملابس نلفها حول greater trochanter وواحد يشد عكس الثاني ونعكد - cervical spine ( ex: comes with collar )needs x ray حتى بعدها اعرف cervical clearance مابيها شي واشيل collar لو لا،، اسويهن حتى لو المريض unstable hemodynamically + foley catheter & measure RBS 3 Fates after that: 1. Death. 2. Compensated. Then 2° survey شلون اعرفه compensated: - normal vital signs - normal urine output : 0.5-1 ml/Kg/hr 3. Not well resiscitated. Bcs of occult bleeding Then damage control S.X ( DCS ) عملية فقط لايقاف bleeding،، لسبب واحد او اكثر مثلا pelvic backing نفتح فتحة ونخلي backs بالداخل ونسد skin حتى نغلق المكان،، او مثلا chest tube او splenectomy,… الخ

+ ال DDH بسيط ومهم وسمنارات د. زكريا هم يجن بالمد.. والاهم من الكلام كله شلون تقرأ X ray ..

الانـدو د.علي طالب يموت ع شي اسمه TX of DKA + ملزمة الـnephropathy بكل الروتات فاقروهن ودرخوهن🤣🤝🏻.
