Topic: Newly found ‘immune organ’ inside skull directs brain defense
Researchers have discovered previously unrecognized lymph node-like immune structures in the skull bone marrow that appear to act as local “first responders” for the brain. In mice, signals from the brain traveled through tiny skull channels to these immune hubs, where B and T cells coordinated antibody responses. Similar immune cells were also identified in human skull bone marrow.
In a glioblastoma model, disrupting these skull immune hubs accelerated tumor growth and reduced survival. Conversely, locally stimulating the hubs enhanced antitumor immune responses and improved survival in mice.
Clinical takeaway: These findings reveal a previously unrecognized system of local brain immune surveillance and could open new approaches for treating brain tumors and other neurological diseases—but the therapeutic findings are currently preclinical.

Topic: Wearables and AI closely track arterial blood pressure in ICU patients
A feasibility study found that a combination of chest ECG, finger photoplethysmography (PPG), and deep learning could reconstruct continuous arterial blood pressure waveforms without an invasive arterial catheter.
Among 28 ICU patients, estimates differed from arterial-line measurements by an average of 6.42 mmHg, with errors of 6.21 mmHg for systolic and 3.41 mmHg for diastolic pressure. The model also generated confidence ranges to indicate when measurements may be less reliable.
The technology could eventually offer continuous beat-to-beat monitoring while avoiding complications of arterial lines such as bleeding, thrombosis, and infection. However, larger real-world validation is needed before clinical use.
Clinical takeaway: AI-powered wearable sensors may provide a future non-invasive alternative for continuous blood pressure monitoring, potentially approaching arterial-line accuracy without the risks of arterial catheterization.

Topic: Doravirine-based HIV regimen provides similar viral suppression with less weight gain
A randomized clinical trial found that doravirine + lamivudine + tenofovir disoproxil fumarate (DOR/3TC/TDF) was noninferior to dolutegravir + emtricitabine + tenofovir alafenamide (DTG/FTC/TAF) for achieving viral suppression at 48 weeks in adults initiating antiretroviral therapy.
While both regimens demonstrated comparable efficacy, patients receiving the doravirine-based regimen experienced significantly less treatment-associated weight gain, an important consideration given growing concerns about weight gain with some modern ART regimens.
Clinical takeaway: Doravirine/lamivudine/tenofovir disoproxil fumarate is an effective first-line ART option that offers similar viral suppression to a dolutegravir-based regimen while being associated with less weight gain.

WHO now suggests paromomycin + miltefosine as the preferred first-line regimen for eligible patients with primary visceral leishmaniasis (VL) in eastern Africa.

- Better tolerated than SSG + paromomycin
- 3-day shorter hospitalization
- Fewer painful injections
