آخر واحدة و اسهل واحدة و ادعولي عشان في حاجة والله 😂❤ Mammary duct ectasia = plasma cell mastitis =comedo mastitis الي هيفكرك بيها إنها قناة و اتسدت و حصل inflammation و خلايا مناعة جات هاجمت ف القناة وسعت.. و حصل fibrosis بعدها يبقى.. ✅dilated duct ✅periductal infiltirate by plasma cells ✅nipple retraction ( slit or transverse مميزة هنا ) برضو في الأعراض معاها الثلاثي ✅mass retroaereolar ✅pain in acute stage only ✅discharge مميزة بتيجي كتير creamy = toothpaste =cheesy أفتكر حاجتين مهمين جدا ال duct papilloma و duct ectasia دول بييجوا في ال major duct و باقي الشابتر في ال TDLU و أفتكر إن ال duct ectasia أشهر سبب ل mammary duct fistula و أخيرا علاجها 3 A ✅Assurance ✅Antibiotics ( flagyl & fluclixacillin ) ✅Anti inflammatory ( steroids ) لو احتاجت جراحة هشيل ال duct اسمها total duct excision أو لو diffuse برضو هعمل cone excision بس افتكر هنا الدكتور الي عملها اسمه هادفيلد. بس كدا جمعتكم مباركة، أظن كدا وضحوا الحمد لله عن الأول 🌷❤

✅Fibroadenosis ( fibrocystic disease) ☆☆the commonest disease in the breast . ✅✅only in reproductive state & hormonal changes & cyclic mastalgia. ✅common bilateral & UOQ & nodules & rubbery in consistency خد بالك fibroadenoma كانت firm .. الكلمة المضيئة Panplasia و أفتكر إنه معاه cyst قد تكون microcyst و ده الأشهر أو macro و دي حاجة مميزة هنا blue dome cyst of bloodgood ✅✅felt by tips of finger ال Fibroadenoma ب ال palm الأعراض؟ أفتكر إن دي معاها الثلاثي ✅mass painful & single or multiple ✅discharge أي لون ما عدا الدم ✅pain dull aching localised أو يسمع للضهر و الدراع و cyclic و لو severe اسمه mastodynia دي بقا معاها Axillary LN بتكون firm tender إيه ال complications? ✅hge in cyst ✅infection ✅carcinoma و نقف وقفة هي في الأصل Precancerous? لو يا دكتور طب امتى يبني ؟ لو .. ✅moderate or severe hyperplasia ✅atypical hyperplasia ✅sclerosing adenosis & macrocyst ال investigations الاسكيم العادية ✅sonomammo.. ✅عندك discharge اعمل cytology ✅biopsy هنا بقا في العلاج قلت كلمة مميزة ☆☆ The main line is conservative Assurance و تخفف الألم و تشجع الحمل و و لو ممكن تاخد أدوية تقلل prolactin و estrogen طب امتى جراحة؟ لو pain مش بيخف و جامد و لو برضو atypical أو suspicion

✅Cystsarcoma phylloid ( serocystic disease) بالأصل soft Fibroadenoma يعني معاه خصائص منها و شوية حجات خصائص منها زي large (10-15 cm ) , mobile , well circumscribed, cystic areas , axillary LN not enlarged .. و شوية حجات زي lobulated ( bosselated ) , consistency variable , solid areas like leaf projections inside cavities معاه حاجة مميزة tear drop sign الي هي extend outside breast و بيعمل cystsarcoma fungoid ايه ال criteria الي تخليني أشك إنه ممكن يقلب mg ? ✅Presence of tumor foci in Margin ✅Cellular atypia & mitotic activity ✅overgrowth in stroma Rare turn mg , metastasis via blood mainly to lung . فيه نقطة كمان مهمة إنه بيعمل pain لما يضغط ع ال nerve لكن مش infiltirative لأنه مش سرطان . العلاج فيه نقطة مختلفة عن ال fibroadenoma إنك هتاخد safety margin بعد ما تشيله حوالي سم أو ٢ سم و اعرف انه poor response to chme , radio امتى أعمل mastectomy ؟ لو massive طبعا أو recurrent

🌷ANDI🌷 أرخم موضوع فعلا فعلا و بيدخل في بعض ف عشان ليلة الامتحان هنبص ع المهم في كل حاجة .. ✅Fibroadenoma ال case بتاعتها واضحة و سهل تتعرف .. painless mobile well circumscribed firm mass و منها نوعين soft أو intracanalicular و ده الوحش الي large و بيكبر بسرعة و cystic و هكذا .. و النوع التاني hard أو peri و ده النوع الحلو ييجي في سن أصغر و أشهر من التاني و هو أشهر breast lump in young female خد بالك painless و no axillary LN أعرف كمان ال pathologic sequences الي هي بتحصل مع ال soft ✅huge / giant ( >5cm) ✅Sarcoma (<1%) ✅Intrapapillary projections ( cystadenoma fungoid) ✅cystic degeneration >>cystedenoma ✅cystsarcoma phylloid (bg or mg.)

Breast 1 .axillary tail of Spence >> deep to deep fascia ( through foramen of Langer) .15-20 openings on nipple ( openings of lactifurus duct of each lobule ) .TDLU ( origin of all pathologies except : duct papilloma , duct ectasia) .Breast bed مهم ( underlying muscles ): mcq Upper 2/3 >> pectoralis major Lower 1/3 ...inner >> rectus ...outer >> external oblique & serratus ant. .Pectoralis minor divide axillary artery to 3 parts , Lymph nodes to 3 levels. .scalenous ant. Divide subclavian a. to 3 parts .Sister Josef nodules , virckow >> advanced breast cancer ( retrograde lymph spread ) فيه اتنين ligaments اتذكروا في المحاضرة : 1. Suspensory ligament ( part of clavipectoral fascia )= coracoaxillary. 2.Halsted's ligament ( costoclavicular) >> condensation of clavipectoral fascia , investing subclavian vessels. #Surgery

Duct papilloma ✅The commonest cause of bleeding per nipple .( more than papillary carcinoma ) ✅Common to transform to malignancy . ✅Samll ( <1cm), not palpable itself . ✅✅Retention cyst . ✅Ttt: if localised >> microdochectomy and biopsy to exclude mg. If not localised >>cone excision of major ducts.

In management of hernia .. you should do U shaped loop of intestine to avoid misdiagnosis of intermediate loop in W / Mydl's hernia و كان قابلني سؤال قبل كدا

The reducible in hernia is the content .. not the sac ☆☆Sac is fixed

No expansile impulse on cough in ▪︎Fatty hernia of linea alba. ▪︎Strangulated hernia.
