Chromosomal Abnormalities – High-Yield Summary
1. Down Syndrome (Trisomy 21)
● Most common chromosomal abnormality.
● Usually caused by Trisomy 21 (95%), less commonly translocation or mosaicism.
● Risk increases with maternal age >35 years.
● Key features:
● Intellectual disability and developmental delay.
● Hypotonia (floppy baby).
● Congenital heart disease, especially VSD.
● Flat facial profile, up-slanting eyes, low-set ears.
● Protruding tongue and short neck.
● Single palmar crease and sandal-gap toes.
● Associated with:
● Leukemia
● Recurrent infections
● Duodenal atresia
● Hirschsprung disease
● Diagnosis: Karyotype = 47,+21.

2. Turner Syndrome (45,XO)
● Occurs in females missing one X chromosome.
● Key features:
● Short stature.
● Webbed neck and neonatal lymphedema.
● Broad chest with widely spaced nipples.
● Delayed puberty and primary amenorrhea.
● Infertility due to streak ovaries.
● Normal intelligence.
● Associated with:
● Coarctation of the aorta
● Renal anomalies
● Hypothyroidism
● Diagnosis:
● 45,XO
● High FSH/LH, low estrogen.

3. Klinefelter Syndrome (47,XXY)
● Male with an extra X chromosome.
● Key features:
● Tall, thin body habitus.
● Small testes and infertility.
● Gynecomastia.
● Sparse body hair.
● Delayed or incomplete puberty.
● Diagnosis:
● 47,XXY
● High FSH/LH, low testosterone.
● Treatment: Testosterone replacement at puberty.

4. Fragile X Syndrome
● Most common inherited cause of intellectual disability.
● Due to CGG repeat expansion (>200 repeats) in the FMR1 gene.
● Mainly affects males.
● Key features:
● Intellectual disability with speech delay.
● Autism-like behavior and ADHD.
● Long face, large ears, prominent jaw.
● Hypermobile joints.
● Macroorchidism after puberty.
● Mitral valve prolapse.
● Diagnosis: Molecular testing showing >200 CGG repeats.

5. Edwards Syndrome (Trisomy 18)
● Second most common trisomy after Down syndrome.
● High mortality during infancy.
● Key features:
● Low birth weight.
● Small jaw and mouth.
● Clenched hands with overlapping fingers.
● Rocker-bottom feet.
● Low-set ears.
● Congenital heart and kidney defects.
● Diagnosis: 47,+18.

6. Patau Syndrome (Trisomy 13)
● Severe chromosomal disorder with very poor survival.
● Key features:
● Cleft lip and palate.
● Microcephaly and microphthalmia.
● Polydactyly.
● Heart and kidney defects.
● Brain malformations.

● Diagnosis: 47,+13.

7. Cri-du-chat Syndrome
● Caused by deletion of the short arm of chromosome 5 (5p deletion).
● Key features:
● Characteristic cat-like cry.
● Microcephaly.
● Round face with wide-set eyes.
● Intellectual disability.
● Hypotonia.
● Cleft lip/palate.
● Diagnosis: 46,del(5p).
