Intubazione orotracheale (orotracheal intubation) is a medical procedure in which a tube (called an endotracheal tube) is inserted through the mouth into the trachea (windpipe) to secure an open airway. This procedure is commonly performed in emergencies, surgeries, or critical care settings to ensure adequate ventilation and oxygenation. Here's an overview:
Indications:
1. Airway protection: To prevent aspiration in cases of decreased consciousness.
2. Respiratory failure: When a patient cannot breathe adequately on their own.
3. Surgical procedures: Especially when general anesthesia is required.
4. Obstruction: Due to trauma, swelling, or foreign body.
Steps:
1. Preparation:
Gather equipment (endotracheal tube, laryngoscope, suction device, etc.).
Pre-oxygenate the patient with 100% oxygen.
Position the patient (typically in a "sniffing" position).
2. Procedure:
Insert a laryngoscope to visualize the vocal cords.
Advance the endotracheal tube through the vocal cords into the trachea.
Inflate the cuff of the tube to secure it in place.
3. Verification:
Confirm placement by auscultation (listening to breath sounds) and capnography (detecting exhaled carbon dioxide).
Secure the tube to prevent displacement.
4. Post-procedure:
Connect the tube to a ventilator or manual resuscitation bag.
