بسم الله Notes on squint (رئيس القسم) •notes of EOM •positive angle alpha means visual axis is nasal to optic axis •binocular single vision (الغاية العظمى للمخ) mans the ability of the brain to obtain or see one image by the use of 2 eyes & it requires similar images & Normal retinal correspondence (fovea) and i develops in 6 years and has 3 stages (simultaneous perception - fusion - stereopsis) •Yoke muscles are 2 muscles in different eyes that have the same direction of action •Herring law: the 2 yoke muscles receive the same amount of stimulatory impulses from the brain when one of them works •Shringten law:when a muscle contracts, the brain sends inhibitory impulses to the antagonist muscles ........................................... •strabismus=abnormal visual axes •Hirschberg test measures the deviation roughly •it's the movemof optic axis that determine the direction of apparent squint •بنحكم على الحَوَل باتجاه الvisual axis وعشان اقول على شخص أنه طبيعي لازم الvisual axes يشاوروا على نفس الobject •eso=nasal ..... Exo=temporal •pseudo=apparent=false •pseudo is the only type of squint with normal visual axes •epicanthus is treated by skin re-distribution .........................paralytic........... •paralytic squint is due to LMNL •squint direction is opposite to the direction of action of the paralysed muscle.. other manifestations are In the same direction of the paralysed muscle •diplopia is binocular ⬅️ طبق بنفسك بقى الclinical picture على كل CN من الجايين دول: ......................3rd CN..................... •the eye deviates laterally only •no diplopia due to complete ptosis (صورة دكتور رافع جفن المريض في اختبار حركة عضلات العين) •posterior communicating artery aneurysm is common cause •ttt: العضلات القوية اضعفها والضعيفة قويها ....................6th CN....................... •common cause is acoustic neuroma (6th palsy+ corneal loss of sensation)& cavernous sinus thrombosis (6th palsy+ Horner $=parkinson's disease) .....................4th CN...................... •worse on opposite gaze (WOOG) & better on opposite tilt (BOOT) ............. heterophoria................... •a state of EOM imblance in which the eye tends to deviate but checked subconsciously by the brain to keep the binocular vision (بدون بلع ريق 🙂) •commonest cause is errors of refraction& most cases are compensated •myopic eye causes exophoria •decompensated cases become concomitant •the squint happens during the examination (cover-uncover) only (داخل سليم وخارج سليم) •SQUINT IS COVER TEST •THE TREATMENT MUST BE IN ORDER .............. concomitant................. •commonest cause of errors of refraction (excess hypermetropia) •squint of the childhood •no diplopia because no binocular vision (غياب الغاية العظمى) ➡️ the brain suppress the diseased eye (الا لو قفلت السليمة ساعتها هيضطر يستعمل دي) ➡️ with time it will become lazy eye (amblyopia) •eccentric fixation= center point other than fovea & less in acquity • worth 4 dots test is very important •cover - uncover test is very important in all types of squint and can differentiate between them (ابحث عنهم بنفسك بقى 🙂) •retinoblastoma can cause concomitant squint so fundus examination is important

في الevisceration بشيل القرنية وبعدين اشيل كل حاجة داخل العين واسيب الsclera and conj and muscles كأني بقور العين من جوة في الenucleation بسيب الconj and muscles بس واشيل العين علي بعضها من غير ما افتحها بعد ما اكون فصلت الconjunctiva and muscles عن العين في الexcentration بشيل كل العين ومعاها الconj and muscles وممكن اشيل الperiosteum اللي مبطن الorbit وممكن كمان اشيل جزء او كل الجفن العلوي والسفلي وفي بعض الأحيان بتشيل جزء من الbones

ده مقارنة ازالة العين بالطرق المختلفة حسب المرض ومكتوب كمان كل طريقة بتشيل ايه من العين وتسيب ايه ميتشالش
