Hematemesis is the vomiting of blood. It is a common symptom of esophageal varices, which are enlarged veins in the esophagus. Esophageal varices can rupture, leading to bleeding. A palpable liver is also a sign of esophageal varices, as the liver can enlarge due to the increased pressure from the blood in the varices.

A 35-year-old man presents with hematemesis. On physical exam, he has a palpable liver. What is the most likely diagnosis?

Explanation: The pericardium is a fibrous sac that encloses the heart and great vessels . It is composed of two layers, the parietal and visceral layers, which are separated by a small amount of fluid that allows the layers to glide smoothly past each other as the heart beats. The fibrous pericardium is the outermost layer of the pericardium and forms a tough, protective sac around the heart. The pericardial sinuses, including the transverse pericardial sinus and the oblique sinus, are spaces formed by the reflection of the visceral pericardium onto the parietal layer. Pericardial effusion occurs when excess fluid accumulates in the pericardial sac, which can compress the heart and impair its ability to pump blood effectively.

What is true about pericardium anatomy in a 55-year-old male patient with chest pain and shortness of breath diagnosed with a large pericardial effusion?
