⚫Procedure of Satellitism Test
1.Prepare a suspension of identified Haemophilus spp. colonies (or suspected Haemophilus influenzae).
2.Prepare a suspension of S. aureus.
3.Using a sterile inoculating loop, streak the suspension in a blood agar medium plate. (Streaking in lines in one direction along the plate will be easy to read.)
4.Using another sterile loop, pick a loopful S. aureus suspension and streak in a line across the inoculated/streaked Haemophilus spp. (Perpendicularly intersecting the lines of inoculation of Haemophilus spp.)
5.Incubate the plate at 35±2°C for about 24 hours at 5% CO2.
⚫Result and Interpretation of Satellitism Test
●The growth of small colonies only in the close vicinity of the S. aureus colonies indicates H. influenzae.
●H. haemolyticus and H. parahaemolyticus are hemolytic in nature; hence, can grow without S. aureus.
●Growth of bacteria across the plate – near and away from S. aureus colonies – indicates Haemophilus spp. that they either need X-factor only or are hemolytic.
⚫Precautions
●Make sure the blood agar plate is sterile and not haemolyzed before inoculation.
●While transferring Haemophilus colonies for making suspension, make sure no piece of chocolate agar medium is transferred.
●Incubate in the presence of 5% CO2.
⚫Applications of Satellitism Test
●To identify isolated Haemophilus spp. as Haemophilus influenzae.
●To differentiate Brucella spp. from Haemophilus spp.
⚫Limitations of Satellitism Test
● It is unable to separate H. influenzae and H. aegyptius.
●H. ducreyi don’t grow in Blood agar even in the presence of S. aureus.
●The bacteria first need to be identified as a member of Haemophilus spp. using Gram staining or other biochemical tests.
● Nutritionally variant Streptococci (NVS) like Abiotrophia defectiva and Granulicatella species, Neisseria spp., and diphtheroid also give a positive result for this test

⚛Satellitism Test:
●The Satellitism test is one of the biochemical tests used to distinguish Haemophilus influenzae from other Haemophilus spp. based on the differential requirement of the X-factor and the V-factor.
●H. influenzae require both X and V-factors for their growth whereas most of the other common pathogenic Haemophilus spp. require only one of the two growth factors
⚫Objectives of Satellitism Test
To identify Haemophilus influenzae.
⚫Principle of Satellitism Test
Blood agar medium provides only an X-factor, but for obtaining a V-factor, the erythrocytes present in the blood agar must be haemolyzed. H. influenzae can neither haemolyze the blood nor grow without the V-factor, so H. influenzae alone can’t grow in a blood agar medium.
●Staphylococcus aureus is hemolytic, and its presence in the blood agar medium makes V-factor (NAD) available in the medium. Hence, H. influenzae can grow in the vicinity of S. aureus colonies in the blood agar medium. This phenomenon is called ‘satelliting
⚫Requirements for Satellitism Test
1. Culture Media
A Blood Agar Medium is used for performing this test.
⚪Composition of Blood Agar Base per 1000 mL
●Beef Heart Peptone – 10.0 grams ●(Alternatively, Meat Extracts – 10.0 grams)
●Peptone – 10.0 grams
●(Alternatively Tryptose – 10.0 grams)
●Sodium Chloride – 5.0 grams
●Agar – 15.0 grams
●Final pH – 7.3 ±0.2 at 25°C
⚫Preparation of Blood Agar Plate
●Measure the appropriate amount of Blood Agar Base powder (40 grams for the above composition) and mix in the water of the required volume in a conical flask or glass bottle according to the instruction of the manufacturing company.
●Stir well using a magnetic stirrer or manually and heat to boiling so that all the components and agar dissolve completely in water.
●Autoclave the flask or bottle at 121°C and 15 lbs pressure for 15 minutes and let it cool to around 40 – 45°C.
●Pour 5% (5 to 10%) v/v sterile defibrinated Sheep Blood into the flask with blood agar base slowly with constant stirring. Mix properly so that blood dissolves uniformly in the medium. This mixture is the Blood Agar.
●In a sterile Petri plate (glass plate with 10 cm diameter), pour around 25 mL of the blood agar and let it solidify properly by leaving it at room temperature.

♦كيفية تحظير السكر التراكمي
يتلخص بثُلُاث خطوات وهي كالتالي:
" العينه تؤخد من اديتا"
1- اول حاجه ناخد 50 ميكرون من الدم + 250 ميكرون من محلول ال lysing وبعد ذلك ننتظر خمس دقائق ليش ؟؟ / لكي يحصل hemolysis
2-ثاني حاجه ناخد 100ميكرون من العينه المحظره في الخطوه اﻻولى ونضعها في انبوب GRT ونضغط 1cm ونعمل mix اي رج لﻻنبوبه لمدة خمس دقائق وبعدين نضغط انبوب GRT اكثر بحيث يخرج لنا العينه صافيه من فوق ونضعها في انبوب صغير ونقرأ بالجهاز وتعتبر هذي القراءه اﻻولى
3-الخطوه الثالثه واﻻخيره وهي اضافه 500ميكرون من DW الماء المقطر + 20ميكرون من العينه المحظره في الخطوه اﻻولى ثم نعمل مزج وبعد ذلك نقرأ في الجهاز
🔴 *مﻻحظه* : طبعا نصفر الجهاز بالماء المقطر ونقرأ قراءة كنترول
واخيرا نحسب القراءه اﻻولى على القراءه الثانيه ونضرب بالفاكتور
♦ بشكل مبسط ( منول)♦
🔷🔷اولا 👈A. Hemolysate preparation
اولا نحضر انبوبتين
الاولى( c)
والثانيه ( T)
وبعدين
نأخذ = 0.5 من المحلول ( lysing Reagent )
الى الانبوبتين
وبعدين نضيف = 0.1 من
( reconstituted control)
للانبوبه =(c)
و = 0.1 من الدم
للانبوبه = (T)
وبعدين Mix للانبوبتين لما يتم التحلل كامل وبيكون بسرعه
وبععدين سوي stand للانبوبتين لمدة 5 دقائق
🔷🔷 الان نجي للمرحله الثانيه 2.
B. Glycosylated hemoglobin ( GHb) separation
هناا . نقوم بأزاله الغطاء من انابين في محلول الفحص نفسه مكتوب عليها ion Exchange resin
ونرقمها نفس الخطوه الاولى
( c) و ( T)
وبعدين نضيف = 0.1 لكل انبوبه من الانابين في الخطوه الاولى
بما يقابلها في الانابين في الخطوه الثانيه
(C) 👈(C) و(T)👈(T)
وبعدين نأخذ انابيب اخرى تسمى Resin separation
وهي انابيب في نفس المحلول تكون فاضيه وعلى رائسها خطاء مطاطي . علشان ندخل هذه الانبوب مع الغطاء اولا علشان هذا الغطاء المطاطي يساعدنا في عمليت الفصل
لنفصل = الطافي من المرسب في نفس العمليه
نقوم بادخال الانبوبه ذات الرأس الماطي في الانابين الاوله في العمليه الثانيه
حتا تكون قريبه للمحلول في الانابيب نفسها. وتكون المسافه 1cm. من المحلول المحلل في الانبوبه بدون ما تلاامس المحلو
وعدين نتركها في ( الراك ) لكي تتم عمليه المزج لمدة 5 دقائق
وبعدهاا يتكون لنا رااااسب sedment . وطافيsupernatent من
المحلو . نقوم بعدها بادخال الانبوبه اكثر الي على راسها مطاط في انبوبه المحلول لكي نعمل حجز وفصل ما بين الراسب والطافي من المحلول
هذه العمليه لكلى الانبوبتين TووC
وبعدها نقوم بصب المحلول. الطافي supernatent
في انابين اخرى
ترقم بنفس الرقم فنصب الTللانبوبهT
وا Cلللانبوبه C
🔷🔷 الخطوه رقم 3
وتسمى = C. Total Hemoglobin ( THb) fraction
وهي كالاتي .
نقوم باضافة =5.0ml من ماء مقطر في انابين اخرى وترقم
(T) و (C)
ونضيف عليهما =0.02mlمن انابين الخطوه رقم واحد A
وبعدين تتم الاقراءه
بطريقه امتصاصيه Ab

هل لون السيرم ممكن استنتج منه حالة المريض ؟
نعم ممكن
١- السيرم الطبيعي يكون لونه اصفر 🟡
٢-السيرم المحمر يدل على تحلل او تكسير كرات الدم الحمراء 🔴
٣- السيرم الذي لونه اصفر مخضر يدل على ارتفاع نسبة البيليروبين🟢🟡
٤- السيرم الذي لونه حليبي وبه عكاره يدل على ارتفاع نسبة الدهون⚪ ..
Differences Between Thick Blood Smear and Thin Blood Smear
♦Thick blood smears
1. Thick blood smears are most useful for detecting the presence of parasites.
2.A thick blood smear is a drop of blood on a glass slide
3.The blood films must be laked before or during staining to rupture all the RBC so that only WBC, platelets and parasites are visualized.
4.Thick smears allow a more efficient detection of parasites (increased sensitivity 11 times than thin smear).
5.It is not fixed in methanol.
6.Thick smears are mainly used to detect infection and to estimate parasitemia.
♦Thin blood smear
1.Thin blood smears helps to discover which species of parasite is causing the infection
2.thin blood smear is a drop of blood that is spread across a large area of the slide.
3.The purpose is to allow malarial parasites to be seen within the RBC and to assess the size of the infected RBCs compared to uninfected RBCs
4.Less sensitive than a thick film especially where there is a low parasitemia.
5.It is fixed in methanol.
6.Thin smears allow the examiner to identify malaria species, quantify parasitemia, and recognize parasite forms like schizonts and gametocytes

👈 لكن قبل ما نتكلم عن الموضوع حابب أتكلم عن الTroponins
والزملاء اللي كانوا بيسألوا عن Troponin C وما فائدته؟
👈 الTroponin Complex عباره عن
Troponin I
Troponin T
Troponin C
والثلاثة عبارة عن بروتينات موجودة في
الCardiac Muscles only not in smooth muscles
Troponin C, also known as TN-C or TnC, is a protein that resides in the troponin complex on actin thin filaments of striated muscle (cardiac, fast-twitch skeletal, or slow-twitch skeletal) and is responsible for binding calcium to activate muscle contraction.
يعني هو مسئول عن الارتباط بالكالسيوم للتحكم في انقباضات عضلة القلب
👈 من وجهة نظري الشخصية أن ليس له أي فائدة تشخيصية وإن كان له فوظيفته الأساسية الارتباط بالكالسيوم للتحكم في انقباضات القلب ويمكن للطبيب معرفة مدى كفاءة القلب وانقباضه وانبساطه عن طريق عمل أشعة إيكو ع القلب
👈 ولذلك تجد معامل كثيرة لا تقوم بعمل التحليل ومن ضمنها أكبر المعامل

1-Myoglobin:
👉It's the oxygen binding low molecular weight protein of cardiac and skeletal muscles cells
هو البروتين الموجود داخل العضلات الذي يرتبط بالاكسجين اللازم للعمليات الحيوية بداخل هذه الخلايا
👉 it's the earliest marker as it rises early after myocardial infarction
هو اول مؤشر يرتفع ف النوبات القلبية
👉 it's not cardiac specific as its levels are raised following myocardial infarction, open heart surgery, muscle injury, muscle dystrophy, renal failure, shock and trauma
هو ليس مميز للقلب لأنه يرتفع ف حالات أخرى
👉rises within 1-3 hours after the onset of symptoms, peaks within 6-9 hours and return to normal level by 1 day
يرتفع ف أول ثلاث ساعات من حدوث النوبة ويصل إلى أعلى مستوى في خلال ٦-٩ ساعات ويعود إلى المستوى الطبيعي ف خلال يوم

2-CK-MB
👉 total serum CK and CK-MB are always elevated following myocardial infarction
يرتفع دائما ف النوبات القلبية
👉 however CK and CK-MB can arise from tissues other than heart
ومع ذلك قد يرتفع لأسباب أخرى متعلقة بالانسجة بخلاف القلب
👉 CK-MB rises within 3-6 hours after the onset of symptoms, peaks within 12-24 hours and return to normal level by 48-74 hours
يرتفع خلال ٣-٦ ساعات بعد ظهور الأعراض ويصل إلى أعلى مستوى خلال ١٢-١٤ ساعة ويعود إلى مستواه الطبيعي بعد يومين او ثلاث أيام
Relative index (RI):
(CK-MB/TOTAL CK *100)
is used to distinguish cardiac from skeletal muscles damage
نستخدمه لتأكيد التشخيص
RI above 5% is highly suggestive of acute myocardial infarction

3-Troponins (Troponin T and Troponin I)
👉 troponins regulate the interaction of actin and myosin filaments during myocardial contraction
هو بروتين ينظم عمل الاكتين والميوسين المسئولين عن انقباض العضلات القلبية
👉 Troponin I is more cardio-specific as it's found only in heart muscles
تروبونين I أفضل من T ف التشخيص
👉 It's not elevated following skeletal muscles injury
لا يرتفع بسبب الاصابات العضلية الهيكلية
👉rises 4-8 hours the onset of chest pain, peaks within 12-24 hours and remain elevated for 7-10 days
يرتفع خلال ٤-٨ ساعات بعد ظهور الأعراض ووجود آلام في الصدر ويصل إلى أعلى مستوى في خلال ١٢-٢٤ ساعة ويعود إلى مستواه الطبيعي ف خلال ٧-١٠ أيام
