Our focus will be on elderly patients, especially those on polypharmacy regimes, and how we can effectively communicate with them, as well as strategies for communicating with children regarding their treatments.
1) Elderly Patients and Polypharmacy :
Elderly patients are becoming a larger portion of our society due to advances in healthcare systems. These patients often are on multiple medications and may have specific communication needs due to age-related changes, such as:
a) Memory Loss: Elderly patients may forget medication instructions quickly. It's vital to write down important information for them.
b) Hearing Impairments: Direct eye contact is essential when communicating, but avoid raising your voice as it may be perceived as shouting.
c) Generational and Ideological Differences: Be mindful of the age gap and differing viewpoints, which can affect communication.
d) Medication Sharing: Many elderly patients share their medications and advise others to take them, which poses significant risks.
e) Speech and Language Difficulties: Conditions like dysarthria (difficulty in articulating words) and aphasia (trouble in understanding or producing speech) are more common, requiring patience and possibly alternative communication methods.
f) Companion Involvement: If there's difficulty in communicating, involve the patient's relative in the discussion.
g) Illiteracy: Use pictograms or visual aids to explain medication instructions to those who cannot read or write.
h) Accessibility: Ensure your pharmacy is accessible to patients in wheelchairs, and communicate at eye level to foster respect and understanding.
2) Communicating with Children
When it comes to pediatric patients, communication strategies differ significantly:
a) Engage Directly with the Child: If the child is over two years old and capable of understanding, talk directly to them about their medication, not just to their parents.
b) Simplify Your Language: Use simple words and sentences that the child can understand.
c) Maintain Eye Level: Sit down to get to the child's eye level, making the interaction more personal and less intimidating.
d) Avoid Frightening the Child: Present the medication in a non-threatening manner to prevent creating any fear or anxiety about taking their medication.

Effective communication in clinical pharmacy is not just about transferring information; it's about building trust, understanding, and a supportive environment for all patients, regardless of their age, physical condition, or cognitive abilities.
