Pk notes Rate and order kinetics 1)First order kinetics هنا عناصر ال ADME بتعتمد علي ال concentration ، مثلا ال elimination rate بيعتمد علي ال concentration of drug في الدم لو كان كبير هتلاقي ال rate of elimination كبييير، والعكس. دا ٩٥ ٪ من الأدوية ال elimination بتاعها بيبقي first order يعني بيتغير بتغير ال concentration وليس ثابت. ال T1/2 هنا بتبقي ثابته مش بتتغير وبنحسبها من المعادله دي T1/2= 0.693/K(elimination rate constant) ال Vd دا ال apparent volume of distribution ودا بيعبر عن ال volume بتاع السوائل اللي ف الجسم اللي الدوا بيتوزع فيها وهو قيمه غير حقيقية... Vd= dose/ cp° بنحسبه زي اي volume بنقسم ال amount ع التركيز بتاع الدوا ف الدم . لما نيجي نرسمه لازم ع ورقه semilog عشان يطلع خط linear ال slope بتاعه negative وبيساوي -k/2.303 وال y intercept بيساوي cp° . 2)zero order kinetics هنا بقي ال elimination rate بيبقي constant ايا كان التركيز فدا مش بيعتمد خالص ع التركيز هو بيبقي قيمه ثابته بتخرج من الجسم كل ساعه . ال T1/2 هنا متغيره ع حسب التركيز . لما تيجي نرسم العلاقه بين التركيز والزمن ع ورقه رسم عاديه بيطلع خط مستقيم نازل من فوق ل تحت وال slope بتاعه negative وبيساوي -K . وال y intercept بيساوي cp° . Bioequivalent Drugs لازم الدوايين علشان اقول انهم bioequivalent لبعض انهم يكون نسخه من بعض ف كل حاجه نفس ال dose نفس التركيز نفس ال dosage form نفس ال chemical form نفس ال therapeutic effect نفس ال pharmacokinetics parameters زي ال c max و T max وال AUC . Bioavailability is rate and extent of drug absorption. فيه منها نوعين Absolute bioavailability بقارن ال bioavailability بتاع دول oral عادي بال bioavailability بتاع نفس الدوا لو اخدته IV. Relative bioavailability بقارن دوايين ببعض ممكن يكونو مختلفين ف ال dosage forms بس نفس ال route of administration واحد منهم مجهول والتاني reference. IV = 100% bioavailability وهي اسرع حاجه ف الامتصاص لانها بتوصل الدم direct. IV infusion kinetics اللي هو المحاليل بندخل الدوا ف الوريد ب rate ثابت يعني مثلاً ١٥ ملي كل ساعه (zero order) لانه ثابت اهو . تركيز الدوا بيفضل يزيد لحد منوصل لل steady state هنا بقي بيثبت التركيز ف الدم لأن معدل الدخول بيساوي معدل الخروج (rate of elimination is first order) . Loading dose دي جرعه تنشيطيه لو عايز اوصل علطول لل steady state بناخد الجرعه اللازمه لكدا ونحقنها علطول IV وبعد كدا نعلق المحلول .
