اول حاجة ف الباثو هي ال Barret's oesophage ‏بص يا سيدي افهم الدنيا ماشيه ازاي علشان تعرف تعبر عندي وعندك وعند كل الناس الطبيعيه الحمد لله ال oesophage بيبقا مبطن ب Stratified squamous epithelium حلو وكمان في صمام كلنا عارفينه من الاناتومي الي هو LES دا بيمنع ان يحصل ارتجاع لمكونات المعدة للمرئ دا شئ عظيم وبالتالي الoesophage مش متعود على ال gastric content اصلا ولا الحامضية دي وطبيعته لا تتحمل فلما يبدأ يتعرضلها تفتكر هيحصل ايه ؟؟ بالظبط هيحاول يحمي نفسه ضد الحاجة الجديده الغريبة الي مش متعود عليها دي فيتحول لنوع تاني من ال epithelium وهو ال Columnar epithelium وعملية التحول دي معروف بنقول عليها metaplasia ✨ معلومة ليك كده اغلب الmetaplasia الي بتحصل بتكون من اي نوع ل Squamous لانه قوي✨ .لكن هنا الامر لله هيتحول ال Squamous ل Columnar لا وكمان مش Columnar عادي دا فيه Goblet cell الي بنقول عليه İntestinal الحوار دا كله علشان اتحول ال Squamous epithelium ل Columnar With Goblet cell الي بنقول عليه✨İntestinal metaplasia ✨ بس يا سيدي هو دا ال Barret's oesophage ببساطه وبيقولك من كتر ال irritation الي بيحصل ممكن يحصل dysplasia وما دام شوفت الdysplasia يبقا خلاص كده داخلين ف مرحلة ال neoplasia طيب دا هيتحول لا ايه بالظبط ال Barret's oesophage progrès to adenocarcinoma لكن ‏the most common malignant in the oesophage is squamous cell carcinoma ‏تمام كده ‏نيجي نشوف الي هنتسأل فيه بقا ‏ ‏اولا .عايز ال Gross pictureلما ابص لل Barret's oesophage دا هشوف ايه الجزء السليم بيبقا pale pink و smooth✨ تنزل لتحت شوية تلاقي ال Barret's ودا بيبقا Dark pink و Velvety mucosa ودا بتبان زي القطيفه كده كأنك بتبص على قطيفه ودا بيبقا بين ال Smooth,pale بتاعة ال Squamousالي ف المرئ وال irregular mucosa بتاعة المعدة لان دا اخر جزء ف المرئ ‏ ‏بسكده 🐣 ‏يبقا ال Gross picture ‏ ‏Dark pink, velvety mucosa between the✨ smooth, pale pink oesophageal squamous mucosa and the more lush, light brown gastric mucosa✨ ‏ أما ال microscopic فانا قولتها قبل كده اصلا ‏Columnar metaplasia with goblet cell ‏(intestinal metaplasia) ‏يا رب تفيد 🙂
