بسم الله الرحمن الرحيم #الاربعاء 19/10/2022 #نافذة_على_الجامعة 🖋محاضرة جراحة🖋 #surgery_lecture السلام عليكم ...🙂 اتأخرنا بسبب الحرس حق مستشفى 48🙂💔 المهم المحاضرة الاولى مع الدكتور علي ضيف الله *abdomenal mass في البداية الدكتور شرح الملزمة ع سلايد سلايد بالتفصيل بس عشان الوقت ضاع ف اضطر انو يختصر الباقي شرح الregions of abdomen بعدها شرح الHx وانو نحلل pain و vomiting بالتفصيل و jaundice وهل جا مع الورم او بعده او قبله و bowel hapite وركز ع الtenesmus وتعريفة  والاEx وركز ع الPR examination وشرحه وقال في العملي اللي ماعيذكره عيرسب 🚶‍♀️ بعدها واصل شرح السلايدات وانواع الtumer والdiagnosisوالاinvestagation واعتذر مننا ع انو الوقت مش كافي يتوسع وقال لنا بمنتهى اللطف والذوق ان اللي معه اي سؤال يساله في اي وقت سواء وجه لوجه او بالوتسب او حتى اتصال🤍🚶‍♀ وبدأنا المحاضرة الثانية مع د فارس الهجامي التي كانت بعنوان الـTrauma والذي بدأ المحاضرة بتوجيه سؤال لـ الطلاب من الذي منكم ماقد تعرض لـ Trauma والبعض فارح رفع يده 😂🌚 اما إحنا مارفعنا لإنه قد تكسرنا لما شبعنا..😂🌚✋ طبعاً موضوع اليوم كان في غاية الأهمية ودخل منه 15 سؤال بالإختبار 🌚💔 من فاته موضوع اليوم فقد فاته الكثير.. اولاً  بدأ الدكتور  بتعريف الـ trauma من أحد الطلاب ثم قام الدكتور بالإجابة #Trauma is: Defined as cellular disruption caused by an  exchange with environmental energy that is  beyond the body s resilience.. 🍃 ثم الـ incidence حيث أخذت الـtrauama  40% من كل الوفيات في العالم للعام الواحد.. الـ trauma لا تعني حوادث السير فقط وانما حتى وخزة الشوكة تعتبر  trauma.. 🌚 ثم شرح لنا ال system الذي يتم من خلاله تقسيم ال trauma على حسب الـ facilities الموجودة في المستشفى كالتالي level 1, 2 , & 3 .. Level 1 – Highest level of care, Leaders in research, clinical  care and education..🍂 Level 2 – Provides definitive care in wide range of complex  traumatic patients..🍀 Level 3 – Provides initial stabilization and treatment..🌸 بعدها انتقل الى آليــــات  حدوث الـ trauma ع حسب نوع الـ trauma هل هي.. 1- Blunt trauma.. 2- compression trauma.. 3- deceleration trauma.. 4- over pressure.. 5- special situation.. 6- penetrating trauma.. سواء كانت.. A- gun shot.. B- stab wound.. C- impalement.. ☘ - بعدها شرح طريقة تقييم الـ trauma ومن ثم طريقة التعامل معها حسب ال severity.. يعني عندما تواجهنا trauma معانا خطوتين نعملها: 1- primary survay.. وهذي تطبق من اول ما تشوف المريض حتى وعاده خارج المستشفى،  والهدف منها انقاذ المريض من اي injury مميت ويكون في ال golden hour..  وتشمل هذه المرحلة ال system المعروف واللي فاهمة اي دكتور وهو الـ ABCDE ( airway,  Breathing)🍁 ثم بعد التأكد من استقرار حالة المريض ننتقل الى الخطوة الثانية: 2- secondary survey.. وفي هذه المرحلة نأخذ  history من المريض او المرافق ونعمل physical examination وفحوصات واشعة وغيرها من الإجراءات الثانوية لكي نكتشف المشاكل الأخرى اللي ما ركزنا عليها في ال primary survay.. 🍂 بعدها انتقل الدكتور الى تفصيل الـ trauma حسب المكان  وابتدأ بال chest trauma: ثم بدأ بتصنيفها الى open and closed بعدها انتقل الى تعريف الـ chest trauma وكيف الية حدوث الـ trauma في الصدر مثل: deceleration, compression..🌸 - بعدها شرح ايش اسباب الـ chest trauma ع حسب نوع الـ trauma.. 🍂 ثم كيف نعمل assessment للـ chest trauma وكما ذكرنا سابقا نبدأ ABCDE يعني اي مريض عندها trauma لازم يمر بهذا السيستم وبالترتيب.. 🌸😊 ثم انتقل الى بعض ال investigations الذي تستخدم في حالة ال. chest trauma مثل ال X- Ray  ,  CT scan , echo  ,  ABG ( arterial blood gases)..🍃 بعدها انتقل الدكتور الى شرح الـ injuries اللي تحدث للـ chest في حالة الـ trauma بالتفصيل وتفريق بين كل واحدة والاخرى من حيث العلامات وال examination وغيرها من الطرق التفريقية.. الـ injuries اللي احدث لل chest: simple pneumothorax  ,  tension pneumothorax  flail chest.. بعدها قام الدكتور بالتحضير وشدد جداً عليه وقال إحنا دفعه خاربه وحضرنا بالورق وقام يعدنا عشان مانغالط ..😂🌚💔 #كمان عطانا حالات نحلهن ونجرب أنفسنا بهن قسم الفريقين اثنين البنات والأولاد والاولاد تقدموا علينا بسؤال ومن الله العوض..🌚😂 بعدها استأنف الدكتور المحاضرة وذلك بشرح الـ trauma of abdomen قال علينا اول عشر ورق منها..  رابط المحاضرات : https://t.me/Surgery_3B/1438 https://t.me/Surgery_3B/1439?single رابط التسجيلات : https://t.me/RecordersOfLectures/456

Renel stone (urethra colic) ..Colitis #Appendicitis ومن ثم بدأ بشرح الموضوع: # Appendicitis:🍂 وشرحة ابتداءً بـ Definition الى الTretment وذلك عن طريق اسئله مباشرة واختيار عشوائي للطلاب.. 😂🌚💔 ومن ثم ركز على شرح #Alvorado_Score وقال ضروري تفهموها صح عشان على ضوئها يتم معرفه ان كان المريض يعاني من التهاب الزائده او لا.. كمان ركز على شرح نقطة الـ leucocyte count.. التي تزداد بشكل moderate وخاصة الـnetrophile وهذا القصد من shift to left..🌿 كما أنه شرح ماهو الفرق: #Shifted pain: appear the pain in primary site and shifted to other site without continuity of pain in primary site.. but... الخميس 13/10/2022 #نافذة_على_الجامعة 🖋محاضرة باطنة 🖋 #Medicine_lecture طبعا اليوم أخذنا 3 محاضرات ب 5 مواضيع 😊 من الساعة ال 8 إلى ال 10 مع الدكتور الرائع أحمد البحيري وكانت المحاضرات كالتالي: * الموضوع الأول: Gastrointestinal haemorrhage طبعا في هذا موضوع بالضبط تحس أنه ما فيش فرق عما درسناه في الجراحة لنفس الموضوع 😁 طبعا الدكتور في البداية سأل عن ال classification والذي ممكن يكون ع حسب ال site بمعنى upper و lower أو ع حسب ال duration بمعنى acute و chronic. او ع حسب ال amount بمعنى overt, occult,...... بعدها انتقل الدكتور الى شرح الاسباب فبدأ بال upper GIT Bleeding وركز ع أنه اهم سبب في اليمن هو ال esophageal varices أما بالعالم الاخر فهي ال peptic ulcer وذكر باقي الاسباب اللي ممكن تؤدي الى نزيف من ال UGIT . بعدها سأل عن Mallory-weiss syndrome وأيش سبب حدوثها. بعدها ركز على ال Dieulafoy's lesion و Cameron lesion وشرح الفرق بينهما. بعدها شرح ال management وكيف نعمل resuscitation للمريض وايضا ال medical treatment وغيرها. * الموضوع الثاني: Constipation ركز في البداية ع التعريف ومتى نقول أن المريض فعلا عنده إمساك وذلك حسب ال frequency of defecation وأيضا ال consistency of stool واشياء أخرى. بعدها شرح ال. epidemiology بأنه يصيب خمس السكان بمعنى واحد من كل خمسة أشخاص وانتقل الى شرح classification بطريقتين إما primary و secondary أو acute و chronic وشرح كل واحد بالتفصيل بعدها انتقل الى ال Causes ع حسب كل نوع. شرح بعدها ال management سواء بتعديل ال Diet و ال laxatives بأنواعها وغيرها. * الموضوع الثالث IBS أو ما يسمى بالقولون العصبي وضح في البداية تعريفه وركز ع أنه functional disease بمعنى أنه مافيش structural abnormalities بعدها انتقل الدكتور الى شرح النظريات التي توضح ال pathogenesis of this disease. بعدها انتقل الدكتور الى شرح ال signs and symptoms والذي أهمها recurrent abdominal pain بعدها وضح الدكتور ال diagnosis وأن أن هذا المرض يشخص بال exclusion يعني نستبعد الامراض الذي ممكن تسبب نفس أعراض هذا المرض. في النهاية تكون الدكتور عن ال management. طبعا الدكتور مركز ع دافدسون وذكره أكثر من مرة بمعنى تذاكروا منه بجمب الملازم 😅 ⭐ محاضرات الدكتور الرائع حِمْيَر قعشة من الساعة 10 وحتى 12:30 الموضوع الأول: * diseases of the stomach موضوع ما في اختلاف خالص عن ما درسناه في الجراحة 😁 في البداية شرح الدكتور انواع ال gastritis الذي ممكن تكون acute أو chronic . بدأ بشرج ال acute gastritis تفصل في ال: - Causes - Symptoms and Signs - laboratory finding - D/D - treatment ال treatment يكون حسب ال cause بعدها شرح ال chronic gastritis وركز فيها ع ال H. It is safe, rapid and effective in almost all cases. 🌺🌿FOLLOW-UP. هذه نقطة مهمة جدا قالت بتأتي في الامتحانات 🌿Routine BHCG follow-up is mandatory for all cases for 1-2 wk. If return to normal within 8weeks follow up to 6month after ERPC. If return to normal after 8weeks follow up to 6month after normal BHCG level. يعني تعمل متابعة بعد العملية من خلالHCG اذا رجعت قيمته الى الطبيعي في اقل من 8 اسابيع نحسب المتابعة من بعد العملية مباشرة لمدة ستة اشهر واذا لم ترجع قيمته الطبيعي الا بعد 8 اسابيع نحسب المتابعة من بعد ان يرجع الى قيمته الطبيعية الى ستة اشهر ثم بدأت المحاضرة الرابعة Renal Disease With Pregnancy Renal Tract infection Cystitis pylonephritis Risk factors: A. Antenatal B.
