بالنسبة لل shunting of syrinx ممكن shunted to subarachnoid space or to peritoneal or pleural cavities بعض الجراحين يجريها بنفس وقت العملية الأولى والبعض الآخر يجريها بعد فترة من العملية الأولى إذا استمر التدهور العصبي بعد العملية

وممكن نعمل هنا معالجات جراحية إضافية باستخدام ال microsurgery بحيث نقدر نعمل Dissection of arachnoid adhesions Opening of foramen of Magendie into 4th ventricle Plugging of the obex

المهم أن المعالجة تشتمل على مبدئين جراحيين 1- Posterior cranial fossa and upper cervical decompression 2- Shunting of the syrinx بالنسبة لل decompression يشتمل على Surgical removal of post rim of foramen magnum, post arch of atlas, and the laminae of the upper cervical vertebrae extending to below the level of decent of cerebellar tonsills And dissection of dural constricting band

أحيانا ال hydromyelia تمتد لجذع الدماغ وتسبب مشاكل إضافية فوق مشاكل الضغط من الأعلى بسبب ال hindbrain herniation

فبالتالي Hydromyelia تكون مبطنة بنفس البطانة الطبيعية لل central canal واللي هي ال ependyma بينما ال Syringomyelia تكون مبطنة ب glial cells

طبعا علشان نفرق باختصار بين hydromyelia and syringomyelia Hydromyelia عبارة عن fluid filled sac connected to central canal بينما Syringomyelia عبارة عن fluid filled sac not connected to the central canal

طبعا كثير من الحالات يكون ال clinical presentation بسبب ال hydromyelia

What are the common anomales associated with Chiari malformation بنقول 1- Supratentorial anomalies هذي تشمل A-Thickening of massa intermedia واللي هي interthalamic adhesision B- Microgyria C- Heterotopias involving both the cereberum and cerebellum 2- Cranial lacunae (Luchenschadel) 3- Anomalies associated with type 2 هذي تشتمل على Cardiovascular malformations Gastrointestinal malformations (imperforate anus) Genitourinary malformations

طبعا هذا الارتباط يمكن تفسيره بميكانزمات معينة ضمن الآلية المرضية أهمها Hydrodynamic theory by Gardner و Differential pressure between intraspinal and intracranial fluid compartments by Williams

من ضمن الأسئلة كانت What are the possible causes of hydrocephalus in a case of Chiari malformation ? هنا بنقول 1- Aqueduct stenosis, atresia ir forking 2- Fusion of sup and inf colliculi bilaterally on both sides 3- Small and crowded post cranial fossa

Type 2 يترافق بصورة كبيرة مع syringomyelia and myelomeningeocele وهذا الارتباط يؤثر على الخصائص الإكلينيكية لهذا التشوه

طبعا هنا بناء على تصنيف Chiari بين 1881-1885 قسم Chiari malformation إلى أربع أنواع Type I وفيه يكون caudal displacement of cerebellar tonsil below foramen magnum into upper cervical canal Type II Caudal displacement of cerebellar vemis, 4th ventricle and medulla oblangata below the foramen magnum وهنا في pathologist ألماني اسمه يوليوس ارنولد وجد ملاحظات مشابهة عام 1894 فتم تسميتها Arnold-Chiari malformation Type III Caudal displacement of the cerebellum and brain stem into a cervical meningeocele وفي التصنيف الأصلي ل Chiari كان في type IV واللي كان عبارة عن حالات hypoplastic cerebellum

Chairi Malformation Downward displacement of the Cerebellar Tonsils ( Narrow foramen Magnum) Compresion of central canal Medulla +associated with Spina bifida ( meningo-mylocele )

Dandy - walker Malformation .. 1- Dilated Forth ventricle ( cystic Dilatation) 2- hypgensis or agensis of Vermis of Cerebellum 3- superior displacement of tentorium + associated with syringomyelia ( #neurology)
