1- her presentation goes with pulmonary embolism and CTPA is the gold standard for diagnosing PE 2- when you auscultate you may hear loud P2 component of 2nd heart sound and pleuritic rub 3- this rash is called livedo reticularis and is seen in patients with : A-Antiphospholipid syndrome (this case is diagnosed as Antiphospholipid syndrome) B-SLE C-Drugs like amantadine 4- you should ask about previous history of DVT , recurrent thrombosis, and obstetrical history as recurrent miscarriages, pre-eclampsia before 34th week of pregnancy

Excellent answers dr shahad Regarding the answers: 1- abdominal distention (ascites), jaundice , clubbing (if you notice it's like drumsticks and he is cachectic[folds in the medial aspect of the thigh]) 2- other findings were hepatosplenomegaly which was +ve in this px in addition to the shifting dullness and spider nevi all which signify cirrhotic px 3- the differential for oedema was just like dr shahd said , but for this case specifically as cirrhosis in this age group DDX could be : Biliary atresia Congenital hepatic fibrosis Cystic fibrosis Alpha 1 anti-trypsin deficiency Caroli's disease Tyrosinemia This infant unfortunately was diagnosed as Caroli's disease And we all wish him good recovery 🙏
