طيب فى حالات ال acute dentoalveolar abscess اقدر اعمل drainage ازاى ؟
Under nerve block anaestheia , you can achieve drainage either by :
*RCT
*intra_oral incision
*through the alveolus "extraction"
بعض الكتب كمان ذكرت انه ممكن تعمل trephination of bone لو كان عندك inaccessible root canal
بس مهم انه ال drainage procedure تكون under antibiotic coverage
وبالتالى هنعطى initial dose of antibiotic by 1_2hours preoperatively
الأمثلة كتير "شخصيا بافضل ال clindamycin تقدر تعطى 600mg clindamycin"
وبعد ما تعمل ال drainage procedure بنعطى maintainace dose of antibiotic لمدة 5_7 ايام
تحديث : قطعا الحديث عن حالات early stage ويمكن لطبيب الأسنان العام التعامل معها ومنع تحول الحالة إلى ما هو أكثر خطورة .. لكن فيه حالات لازم تتحول لجراح فم ووجه وفكين .. قطعا فيه criteria متفق عليها علشان نقول ان الحالة تتحول لجراح الوجه والفكين

It is rare lesion of uncertain histogenesis that occurs exclusively in newborn infants, chiefly on the maxillary anterior alveolar ridge and less commonly on the mandibular anterior alveolar ridge. Although usually solitary lesions, they may be multiple, most often affecting both the maxilla and mandible, although rare simultaneous occurrence of lesions on the maxillary anterior alveolar ridge and tongue have been reported
Sometimes it may resolve spontaneously..but In some cases it may be large enough to cause respiratory problem or even asphyxia or may interfer with sucking, in such cases surgical removal will be planned .If general anesthesia will be used, discussion with pediatrician and anesthetist is a must..
