🔷 It depends on Mentzer index = MCV/RBC:
🔻<13: thalassemia
🔻>13: IDA
• Whether I have ID or thal trait, I want to optimize my Hb/Hct (evolution selects for an optimal Hb/Hct, not for red cell number or size)
• Hct = MCV x RBC count, so if my MCV falls (say in ID or thalassemia), my bone marrow "wants" to compensate by making more RBCs (via increased EPO)
• In thal trait, my bone marrow is up to the task. It makes more RBCs than normal, maintaining a close to normal Hb/Hct
• In ID, my marrow is missing an essential nutrient for erythropoiesis; thus, RBC count and Hct are lower for the degree of MCV (despite EPO drive)
• ID is associated with a wider RDW than thalassemia
