PALATE PERFORATION DUE TO COCAINE USE
The use of cocaine via inhalation (the best known form of administration) can lead to βcocaine-induced destructive midline lesionsβ. A classic and uncommon example is perforation of the hard palate.
The mechanism of inflammation and necrosis is multifactorial, which includes the local ischemic vasoconstrictor effect (cocaine is a vasoconstrictor substance), local trauma, mucosal irritation due to the association with other substances, and also secondary infection.
Diagnosis is made through a detailed history and clinical assessment, and can be challenging, as many patients deny using the drug. Some differential diagnoses may be Tertiary Syphilis, Wegener's Granulomatosis and Malignant Neoplasms.
The patient has stopped using drugs and is using an obturator prosthesis to help with speech and eating. The HCPA craniomaxillofacial surgery team is monitoring the case and plans surgical reconstruction for definitive treatment.

Immunotherapy with checkpoint inhibitors, such as pembrolizumab, has transformed cancer treatment in the last decade, improving survival rates for several types of advanced tumors. Treatment-related adverse events are common and can affect any organ system. Oral manifestations can range from asymptomatic white streaks to potentially fatal mucocutaneous reactions requiring aggressive treatment with corticosteroids and/or permanent discontinuation of immunotherapy. Oral mucositis results from toxicity is one of the most common side effects of radiotherapy and chemotherapy, in cancer treatment and for hematopoietic stem cell transplantation. Clinically, these alterations are characterized by epithelial atrophy, edema, erythema and the appearance of ulcerations, which can affect the entire oral mucosa, causing pain and discomfort, impairing speech, swallowing and feeding.

SQUAMOUS CELL CARCINOMA. Yet another tragic case of professional negligence!! The patient had a consultation with two ENTs and two Dentists complaining of pain in the tongue in the last three months. No one performed a biopsy or instructed the patient to see a specialist. Clinical examination showed a rather large ulcerated lesion with evident areas of necrosis, extremely fetid odor and palpable submandibular and cervical lymph nodes (indurated and fixed). What's the prognosis? Probably very badβ¦.. An illness that could have been successfully treatedβ¦. Sad.
