A 15-year-old girl went to see a dental surgeon with a referral from her family dentist. The patient was worried about the lesion on the mucous membrane of the left cheek. According to patient, this lesion was present since a year. Initially, the damage was small, but began to grow after repeated biting injuries.
The Patient's history was marked by Increased Anxiety.
Extraoral examination was within the normal range. An Intraoral Examination revealed a Soft, Painless, Red Pedicle Lesion.

The photo shows hypertrophied foliate papillae of the tongue, located on the lateral surfaces on both sides. They are covered with non-keratinized epithelium and contain many taste buds.
At the base of the papilla is lymphoid tissue, which is what leads to their hypertrophy. The reaction may be caused by external stimuli, infection, trauma, smoking, or other factors.
Hypertrophic Foliate Papillae are often confused with neoplasms by both patients and clinicians.

The Nikolsky sign is a clinical sign seen in certain skin and mucosal conditions where the skin or mucous membranes near active lesions can be easily separated from the underlying layers when gentle pressure or lateral shearing is applied.
This can be tested by pressing or rubbing the skin/mucosa near active lesions or using an air jet on the skin/mucosa to see if they peel off easily. The sign is caused by a weakening of the skin or mucous membranes and is seen in conditions such as pemphigus vulgaris, which is an autoimmune disorder characterized by blistering lesions.
In this case, An Air Jet is used to produce the same effect, there is formation of a bubble due to fragility of the epithelial layer.
However, Nikosky's sign is not pathognomonic and can be observed in some vesiculobullous diseases like pemphigus vulgaris, toxic epidermal necrolysis, Stevens-Johnson syndrome, and other blistering disorders.
