Peripheral ossifying fibroma is a reactive lesion of fibrous connective tissue, in which there are foci of mineralized material supposedly originating from cells of the periosteum or periodontal ligament. Although its pathogenesis is uncertain, it is possible that this lesion is a response to chronic irritation or low-intensity local trauma. Clinically, it presents as a nodular, well-defined lesion, with a sessile or pedunculated base, slow growing, reddish in color or similar to that of the mucosa. This fibroma has a predilection for white female patients, between the second and third decade of life and located almost exclusively in the gums, in the anterior region of the maxilla. Histopathologically, a stratified squamous epithelium is observed, which may be ulcerated or not, covering the vascularized fibrous connective tissue with multiple fibroblasts, deposition of collagen fibers and mineralization in the form of cementum globules or cementicles, osteoid, mature bone and dystrophic calcification. Commonly, there is a combination of these mineralized materials. If the lining epithelium is ulcerated, its surface will be covered by a fibrinopurulent membrane with an area of ββunderlying granulation tissue.
The final photo was taken 1 month after removal of the lesion, with no signs of recurrence. Patient remains under follow-up.
