When questioning the patient, it was revealed that before the last appointment with the dentist, which took place 1.5 months ago, there was a rather long break - almost two years. During this break, she suffered a heart attack and stroke. The patient also had a history of hepatitis C, due to which the patient was placed on the liver transplant list.
The medications taken included furosemide, escitalopram, ursodiol, omeprazole, baclofen, spironolactone, bupropion, potassium, trazodone, gabapentin, lactulose, and estradiol. Just three weeks before the visit, she began taking the drug Harvoni, used to treat chronic hepatitis C.
Given the impressive list of medications taken, it has been assumed that there is a high risk of developing dry mouth , which in itself puts people at risk for developing oral diseases. In addition, one of the side effects of the drug Harvoni is elements of lesions in the oral cavity, and in this case - in combination with dryness - these lesions only worsened. Since this drug was mandatory for the patient, given her medical history (for three months), a palliative approach was chosen.
The dentist held a consultation with the attending infectious disease specialist to agree on management tactics for the patient. The patient was given a prescription for βMagic Mouthwash,β which is usually prescribed to patients suffering from mucositis during the treatment of cancer. In this case, the mouthwash consisted of three ingredients mixed in equal proportions (1:1:1): a 2% viscous solution of Lidocaine, an Antihistamine (eg, Benadryl), and an Antacid (Maalox).
All invasive procedures were delayed until symptoms resolved. The patient was given detailed instructions for oral care at home.

A 64-year-old woman came to the clinic for Oral Prophylaxis. Before treatment, she said that she was worried about formations like ulcers in the mouth. The lesions increased in size, making it difficult for the patient to drink and eat.
On examination, hyperemia of the oral mucosa as a whole was noted. The most noticeable ulcerations were on the lateral surfaces of the tongue on the right and left (shown in the photo). The edges were clearly defined and the central part was slightly concave and very sensitive to palpation. In addition, the tissue easily peeled off and bled at the slightest touch. Angular cheilitis was observed, opening the mouth was painful (shown in the photo).

Nasopalatine Canal (NPC)
The nasopalatine canal is a relatively broad bone channel originating from the anterior nasal floor and terminating at the incisive foramen in the anterior palate. The nasopalatine artery and nerve running through the nasopalatine canal connect the sphenopalatine arteries and nerves with the greater palatine arteries and nerves. In fact, they represent vascular and neural anastomoses that are often found in the orofacial region.
An interesting structure associated with the nasopalatine canal is the nasopalatine duct that is often misdiagnosed as a pathological sinus tract.
The nasopalatine canal (NPC) is a bone channel connecting the nasal cavity and the palate, hence its name. Since the NPC is located palatal to the central incisors, it is also referred to as the Incisive Canal.
The NPC is not to be confused with the nasopalatine duct, an epithelialized structure within the NPC.
The NPC terminates with a palatal opening, the so-called Incisive Foramen.

One of the causes of Chronic Macroglossia in adults is AMYLOIDOSIS.
It is the accumulation of a Protein (Amyloid) capable of being deposited in different tissues and organs for various reasons. The problem is that the accumulation of Amyloid is capable of altering the function of the Organ, putting the patient's life at risk, especially when these are vital organs such as the Heart π«or the Kidney.
The diagnosis of oral lesions is made by Biopsy and staining with Congo Red, in which the classic finding of Birefringence is observed π with Polarized light.

Mucocele of the Blandin-Nuhn glands
Mucocele due to extravasation phenomenon is a lesion frequently found in the oral mucosa of children. Its etiology is generally associated with local trauma, resulting in rupture of the excretory duct of accessory salivary glands and promoting mucin spillage into the surrounding soft tissue. In the ventral region of the tongue, it involves the Blandin-Nuhn glands, and is clinically characterized as a dome-shaped swelling of the mucosa, soft to palpation, with a smooth and painless surface, with a color ranging from the normal pink of the mucosa to blue.
In this case, the patient was 18 years old, and reported an evolution time of 1 month, with progressive volume increase. The treatment performed was excisional biopsy.
