Acute respiratory distress syndrome ⭕️Definition: 🌀acute respiratory insult or worsening in respiratory status within 1 week. 🌀 bilateral infiltration without alternative explanation such as effusion, nodules or atelectasis. 🌀Edema not fully explained by fluid overload or CHF. 🌀Hypoxemia. ⭕️Etiology: 🌀direct ➡️ pneumonia, aspiration, inhalational injury, lung contusion and near drowning 🌀 indirect ➡️ sepsis, shock , DIC, pancreatitis, TRALI , trauma. ⭕️ Treatment: the 6 Ps ⁃ PEEP. ⁃ Prone position . ⁃ Paralysis( not routinely recommended unless dyssynchronised ventilation) . ⁃ Peeing ( fluid balance) target Cvp 2-4 cm H2o ⁃ Pulmonary vasodilator such as NO. ⁃ Perfusion( v-v ECMO) maybe useful if refractory.

Finding and management ?

One of the cases i saw in the ER as a stagiaire… 50 years old male a known case of HTN / DM / IHD presented with 3 hours hx of chest pain , vomiting and sweating OE/ The Bp was 80/50 at the beginning then become undetected neither PR ECG was done below 👇🏻 Cardiac centre refused to take the patient until he become stabalized with sinus rythem 2 DC shock was given without improvement The patient received Aspirin Plavix Heparin Doputamin Tramal and diazepam The patient unfortunately passed away after a while What do you think about the ecg?

هذا الحجي اجه عنده يشكي من صداع فقط وشكله ستيبل جداا جداا جيكت الفايتل كله تمام ، اجه بوقت كلش لود بس دائما اسال ع chest pain وادز اي واحد كبير بالعمر ع ECG مهما كان البرزنيشن مالته , كلي اي صار عندي الم خفيف بصدري دزيته ع تخطيط وطلع inferior MI , دخلته انعاش وكفرتله انتي اسكيميا وسلمته لجماعة الشفت الليلي .
