Where third parties are exposed to a risk so serious that it “ outweighs the patient's privacy interest “, you should “ seek consent to disclosure “ where practicable. Afaq Omer Ahmed You are absolutely right‼️ What a sad 😔 tragedy but let me make it straight and tell you about other hidden causes of death 💀 and highlights the gaps for quality improvement such as: Suboptimal adherence to the key recommendations from the Global Initiative for Asthma (GINA) guidelines for management asthma guidelines. These were: (1) The prescription of oral and inhaled corticosteroids (OCS and ICS) and issuance of outpatient follow-up for patients discharged from the ED. (2) Low adherence to Asthma Clinical Practice Guidelines. why❓ (A) Lack of Knowledge on Clinical Guidelines: why❓ - High staff turnover of frontline ED junior doctors. - Guidelines are lengthy and frequently updated. - Difficult to read and recall. - Low doctor attendance for asthma continuing medical education activities due to shifting work. (B)Processes. Why❓ - ED doctors are not sure whether to refer patients for primary or specialist follow/-up care - No discharge checklist available to remind' ER doctors and nurses of best practices - No Asthma pathway available to remind' ER doctors and nurses of best practices. - No audit system in place to monitor the compliance rate to asthma clinical guidelines. (C) Materials. Why❓ - lack of educational aids and an inhaler training devices - No spacer available in the ED pharmacy (D) After-hours Environment. Why❓ - More asthma patients present at night. - Less ED doctors and nurses on duty. - No specialist asthma nurse on duty.
