Recommendation for levothyroxin therapy in pregnant women with subclinical hypothyroidism 💜 #ATA

لو pregnant وكانت preexisting Hypothyroidism وبتاخد L-Thyroxin  هل  جرعته هتختلف خلال ال pregnancy 🤔: 🔹 خلال ال pregnancy بيزيد احتياج الجسم لل T4 وال Goal تبعناا لل TSH هيبقى Between Lower limit reference - 2.5mu/L والافضل يكون اقل من 1.2 عشان هيك جرعة ال levothyroxin هنزودهاا 20-30% عن جرعة ما قبل الحمل 🔸#ATA Recommendation : for pregnant women with existing hypothyroidism, increase levothyroxine dose by 20%-30% (can be achieved by taking 2 additional tablets weekly) as soon as pregnancy confirmed (ATA Strong recommendation, High-quality evidence) يعني لو هي بتاخد 7doses خلال الاسبوع هنخليهم 9doses , فهتاخد يومين Double dose وتكمل الايام الاخرى ب single dose ⬇ We typically accomplish this by increasing the dose from once-daily dosing to a total of nine doses per week (double the daily dose two days each week).

فيميل عدها Hypothyroidism  وتاخذ Thyroxine و صارت pregnant  ياترى يحتاج نسوي شي ؟! واكو شي مميز بالـ thyroxine tab 🤔 .. ⚫️ الفيميل من تصير pregnant حينفرز شي اسمه placental deiodinase  بالاخص الـ type III هذا وظيفته يسويلي  increases clearance of T4  وبالتالي الـ Thyroxine  حيقل بالجسم - وايضا حتصيرلي عده اشياء وهي↙️↙️ - transfer of T4 to the fetus - high serum thyroxine-binding globulin (TBG) concentrations - weight gain and increased T4 pool size كل هاي الامور ↖️↖️  حتبدي تستهلك بال Thyroxine مال الام   الي كان نورمل قبل متصير Pregnant وهذا حيؤدي انو ال TSH يرتفع وتبدي تتغير القراءات كلها وتصير الام Hypothyroidism  !! ⚫️ الـ hypothyroidism وبالـ pregnancy  بي هوايه compilcation  سواء على الام نفسها او على الطفل وعقليته !! وحتى علـ IQ مال طفل !! ومن ضمن هاي الـ compilcation  ↙️↙️ - Preeclampsia and gestational hypertension - Placental abruption - Nonreassuring fetal heart rate tracing - Preterm delivery, including very preterm delivery (before 32 weeks) - Low birth weight (which was likely due to preterm delivery for preeclampsia in one study but not in a second study where the rate of preeclampsia was negligible - Increased rate of cesarean section - Postpartum hemorrhage - Perinatal morbidity and mortality 🔴Neuropsychological and cognitive impairment in the child 🟣 فحتى نجنب هل مشاكل فصار two approaches  حتى نعدل الوضعيه  - الـ Uptodate  يكول ↙️↙️ - hypothyroid women who are newly pregnant should preemptively increase their levothyroxine dose by approximately 30 percent and notify their clinician promptly .. 🟤 هل شي ياترى شلون يصير ؟! 🤔 مثلا لو وحده تاخذ 100mcg يوميا يعني بالاسبوع تاخذ 700mcg فاذا نريد نزيد الجرعه 30% يعني نضرب الـ 700 بالـ 30% حيطلع الناتج 210  فتصير الجرعه الكامله الي لازم تاخذها 910mcg وخلي نكول تقريبا  900mcg فهسه شلون حنكلها تاخذ الجرعه ؟! 🧐 - الحلو بالـ Levothyroxine  انو الـ half life مالته اسبوع وهل شي يسمحلنا انو نرتب وقت الاعطاء بسهوله فهنا نكول للام  يابه اخذي جرعتج نفسها شلون تاخذيها مثل كل يوم والي هي  100mcg صباحا قبل الاكل بساعه بس اخر يومين بالأسبوع اخذي حبايتين ام الـ 100mcg بدل حبه وحده ، وبالتالي حصلنا على 900mcg اسبوعيا مثل مردنا .. وصلت الفكره ؟! (( بعدها نبدي نجيك الـ TSH كل 4 اسابيع حتى نعرف احنا وين عن  التاركت ))

🛑Sickle cell disease  in pregnancy ✅Aspirin 75mg od should be considered for women with SCD from 12 weeks gestation, aiming to reduce the risk of pre-eclampsia. ✅ SCD carries a higher risk of VTE. This should be included in the VTE risk assessment for these women and thromboprophylaxis prescribed as appropriate. ✅ LMWH thromboprophylaxis will usually need to start at 28 weeks gestation but may need to be started prior to this if additional VTE risk factors exist. ✅Thromboprophylaxis should be administered (if not contraindicated) whilst in hospital and for minimum of 7 days following vaginal delivery or 6 weeks following C-section.
