When ADP binds to its platelet receptor, it leads to activation of glycoprotein IIb/IIIa complex and eventually results in platelet aggregation.
Clopidogrel first gets metabolized by CYP450 enzymes to the active form which then selectively inhibits the binding of ADP to its platelet receptor, thereby preventing platelet aggregation.
Newer medications like ticagrelor have a similar mechanism of action as clopidogrel.
Abciximab is an example of a glycoprotein IIb/IIIa inhibitor.
Eptifibatide is an example of glycoprotein IIb/IIIa antagonist.
Dipyridamole acts in multiple ways, one being inhibition of platelet cAMP-phosphodiesterase. It then increases intra-platelet cAMP, which leads to reduced arachidonic acid release.
It finally results in decreased thromboxane A2 formation.
