Sudden, painless visual loss (often monocular)
Amaurosis fugax: transient βcurtain coming downβ β vascular cause
Optic neuritis: painful eye movements, reduced colour vision
*2οΈβ£ Stroke (Occipital Lobe)*
Sudden visual field loss (homonymous hemianopia)
Vision loss is often bilateral but symmetric
May have other focal neurological signs
*3οΈβ£ Giant Cell Arteritis*
Visual loss in patients >50 years
Associated with headache, jaw claudication, scalp tenderness
Medical emergency β start steroids immediately

C β Coronary artery disease (ACS / Angina)
β Central, pressure-like pain Β± radiation, associated with exertion or risk factors.
A β Aortic pathology (Dissection)
β Sudden severe βtearingβ pain radiating to the back, BP differential.
R β Reflux / Oesophageal spasm
β Burning retrosternal pain, post-prandial, worse lying flat.
D β Diseases of lung (PE, Pneumothorax, Pneumonia)
β Pleuritic pain, breathlessness, hypoxia.
I β Inflammation (Pericarditis)
β Sharp pain, worse on inspiration, relieved by sitting forward.
A β Anxiety / Panic attack
β Chest tightness with palpitations, normal investigations.
C β Chest wall / Musculoskeletal
β Localised, reproducible pain on palpation or movement.
P β Pleural causes
β Sharp pain with respiration, often unilateral.

Often associated with weakness or visual symptoms

*V β Vasovagal*
β Reflex hypotension/bradycardia triggered by pain, fear, prolonged standing.
*P β Postural (Orthostatic hypotension)*
β Drop in BP on standing due to dehydration, autonomic failure, or drugs.
*A β Arrhythmia / Aortic stenosis*
β Sudden reduction in cardiac output causing transient cerebral hypoperfusion.
*S β Situational syncope*
β Coughing, micturition, defecation, swallowing (vagal reflex).
*S β Seizure / Subclavian steal (mimics)*
β Important differentials that can present like syncope but need exclusion.

( bacteria viruses, parasites and Fungus)
are BUGS .
The word INFECTION is tolerable ( not considered as a Jargon by most of the examiners)
so u will say to your patient that you have bug infection ( in your lungs, tummy whatever etc)
