Comments: Benign giant cells resembling osteoclasts are seen in about 25% of osteosarcomas. In rare osteosarcomas, benign giant cells are so numerous that they may obscure the malignant cells in the background and lead to the mistaken diagnosis of a giant cell tumor. This is more likely to occur in sacrum - a frequent location for giant cell tumor. The radiographic differences between the giant cell tumor and osteosarcoma are less apparent in the sacrum than in the long bones. It is worth remembering that usual giant cell tumor occurs in skeletally mature patients. A tumor that appears to be a giant cell tumor but arising in a skeletally immature person should be sampled thoroughly and carefully to exclude osteosarcoma rich in giant cells.

Comments: There are many classification systems for grading osteosarcoma. One of the widely used system is the 4-tiered (Broder's) system based on the percentage of anaplasia in the tumor. Majority of the conventional central osteosarcomas fall in grade 3 or grade 4 category. The tumors assigned grade 1 are low-grade central osteosarcomas and the usual parosteal osteosarcomas. Osteosarcomas of the jaw often tend to show grade 2 histology. The image shows a focus of grade 4 osteoblastic osteosarcoma.

Comments: Approximately 25% of osteosarcomas consist predominantly of spindle tumor cells and can be classified as fibroblastic osteosarcoma. Osteoid production is minimal and seen focally; although in this case, osteoid matrix was readily found. Fibroblastic osteosarcomas are highly vascular and may resemble hemangiopericytoma.

Comments: Lace-like osteoid deposited in between frankly anaplastic tumor cells. The patient was a 13 y/o boy who presented with history of pain around his right knee joint. Plain films and MRI scans showed a lesion in upper tibia that was diagnostic of osteosarcoma. Final diagnosis was Grade 4 osteoblastic osteosarcoma

Comments: In chondroblastic osteosarcomas, the cartilaginous areas are often arranged in lobules. The center of the lobules are less cellular than the periphery (shown on the lower left). Islands of osteoid matrix are seen within the cartilage. The periphery of the cartilaginous areas show condensation and spindling of the tumor cells.

Comments: In chondroblastic osteosarcomas, the tumor cells are present in lacunae and arranged in lobules. The periphery of the lobules is hypercellular and shows condensation and spindling of the tumor cells. Osteoid matrix is seen on the lower right of the image.

Comments: Chondroblastic osteosarcoma involving proximal tibia in a 15 year old male. Bluish-white areas of cartilaginous differentiation are scattered throughout the tumor. Microscopically, these tumors show malignant-appearing chondrocytes in lacunae, crowded areas of spindle cells at the periphery of the lobules, and islands of feathery osteoid matrix within the cartilaginous areas. About 20% of osteosarcomas are classified as chondroblastic.
