⭕️الأسباب الشائعة لحدوث ال
(Hypothyroidism)
✅Primary Hypothyroidism
➡️Chronic autoimmune thyroiditis (Hashimoto disease)
➡️Iatrogenic (irradiation, surgery)
➡️Drugs (amiodarone, radiocontrast media, lithium, interferon-α,
tyrosine kinase inhibitors, immune checkpoint inhibitors)
➡️Transient thyroiditis (including viral and postpartum)
Iodine deficiency and excess
Thyroid gland infiltration (infectious, malignant, autoimmune,
inflammatory)
✅Secondary Hypothyroidism
➡️Pituitary disease
➡️Hypothalamic disease

🔸2nd line lamotrigine,Valporic acid
🔸3rd line➡️ levetiracetam
✅في حالة ال generalized tonic clonic siezer
🔹1st line➡️ Valporic acid
🔹2nd line➡️ lamotrigine, Carbamazepine
🔹3rd line➡️ levetiracetam
✅في حالة ال Childhood absence seizure
🟨1st line➡️ Ethosuximide
🟨2nd line➡️ Valporic acid, lamotrigine
🟨3rd line➡️ clonazepam
✅في حالة ال myoclonic seizure
🔘1st line➡️ Valporic acid
🔘2nd line➡️ lamotrigine

أفضل الأدوية هي
👁🗨lamotrigine or
👁🗨levetiracetam
✅في حالة ال status epilepticus
◻️1st line ➡️Diazepam IV (adult)
rectal (children)
◻️2nd line➡️ fosphenytoin, levetiracetam
😃ولا تنسى أنك يجب أن تحرص على إستخدام دواء واحد فقط إلا إذا توجب الأمر بإمكانك أن تعمل compination..😉

⭕️عندما ال monotherapyالذي يستخدم ك first-line
يفشل في معالجة الصرع
✅يجب أن تحاول إستخدام العلاج الأول مع الدواء الثاني..
✅أيضاً يجب أن تعيد الفحص قبل البدأ بال
alternative drug if the first drug showed lack of efficacy.
✅يجب أن تعلم بأن التغيير من دواء إلى آخر من أدوية الصرع يجب أن يكون بحذر..
نسوي slowly withdrawing
للدواء الأول إذا قررنا نستخدم الدواء الجديد،
Combination therapy with two or more antiepileptic
drugs may be necessary,
✅لكن الإستخدام المتزامن لأدوية الصرع يزيد من خطورة ال
adverse effects and drug interactions.
لذلك ال
combination therapy
إذا لم يأتي بالفائدة المرغوبة.
revert to the regimen (monotherapy or combination therapy) that provided the best balance between tolerability and efficacy.
🙂A single antiepileptic drug should be prescribed wherever possible.😐

⭕️ليكن بعلمك أن الهدف من معالجة ال epilepsy منع حدوث ال seizures بواسطة المحافظة على الجرعة المؤثرة سواءكان دواء واحد أو أكثر
✅لذلك العناية بضبط الجرعة شيءضروري..
⬅️أي نبدأ بجرعة منخفضة ونزيدها gradually حتى نسوي controled لل seizures.
✅تكرار الجرعة غالباً تحدد بواسطة 👇
plasma-drug half-life
لذلك معظم ال antiepileptics
عندما تستخدم في الجرعة اليومية، ممكن أن نعطيها مرتين يومياً ،
⬅️ماعدا بعض
الأدوية مثل ال Lamotrigine, perampanel, phenobarbital, and phenytoin
✅تعتبر long half-lives
تعطى مرة واحدة يومياً عند النوم
لكن 😐👇
with large doses, some antiepileptics may need to be given more frequently to avoid adverse effects associated with high peak plasma-drug concentration.

⭕️تذكر بأنه عندما نختار ال
antiepileptic drug
✅يجب نأخذ بعين الإعتبار كأول خطوة وجود ال
epilepsy syndrome
✅إذا ال the syndrome غير واضح
⬅️نوع ال seizure هي اللي تحد ال
the choice of treatment.
✅أيضاً👇😐
Concomitant medication,
co-morbidity, age, and sex should also be taken into account.

⭕️ When used the aspirin in pregnancy and what's contraindicted ?
✅Contraindiction: because Use of aspirin close to delivery may cause premature closure of the ductus arteriosus. Adverse effects reported in the mother include anemia, hemorrhage, prolonged gestation, and prolonged labor .
✅Used in low dose in the following?
➡️1-Low-dose aspirin may be used to prevent preeclampsia in women with a history of early-onset preeclampsia and preterm delivery (<34 0/7 weeks), or preeclampsia in ≥1 prior to pregnancy ) Treatment is started after 12 weeks' gestation in women at risk for preeclampsia .
✅2- Low-dose aspirin is used to treat complications resulting from antiphospholipid syndrome in pregnancy (either primary or secondary to SLE)
➡️3-Low-dose aspirin to prevent thrombosis may also be used during the second and third trimesters in women with prosthetic valves(mechanical or bioprosthetic).
➡️4-The use of warfarin is recommended, along with low-dose aspirin, in those with mechanical prosthetic valve .
➡️5- Low-dose aspirin may also be used after the first trimester in women with low-risk conditions requiring antiplatelet therapy.
