A 15-year-old male develops severe cardiomyopathy after contracting an infective myocarditis from the Coxsackie virus, leading to his placement on the cardiac transplant list. Two weeks post-transplantation from a matched donor, he experiences dyspnea upon exertion. Extensive evaluation, including cardiac catheterization and endomyocardial biopsy, is conducted. Which of the following findings best indicates acute graft rejection? A. Concentric coronary atherosclerosis B. Dense interstitial lymphocytic infiltrate C. Perivascular infiltrate with abundant eosinophils D. Patchy necrosis with granulation tissue E. Scant inflammatory cells and interstitial fibrosis Answer: https://bit.ly/3vAkkkm

A 55-year-old woman with a history of hypertension and type 2 diabetes mellitus presents with a 6-month history of progressive skin darkening, particularly on her knuckles, elbows, and knees. She also reports increasing fatigue, muscle weakness, and difficulty climbing stairs. Upon examination, she exhibits mild proximal muscle wasting and her potassium level is found to be low (2.8 mmol/L) Investigations: Serum cortisol: elevated (4 times the upper limit of normal) 24-hour urinary cortisol: markedly elevated Dexamethasone suppression test (low dose): no suppression Based on the clinical presentation and initial investigations, which of the following is the most likely diagnosis for this patient's Cushing's syndrome? A. Ectopic ACTH secretion B. ACTH-secreting pituitary adenoma C. Adrenal adenoma D. Iatrogenic Cushing's syndrome

Why are children more vulnerable to biologic agents than adults? Anatomic and physiologic differences: Thinner dermis, increased surface area-to-volume ratio, smaller relative blood volume, higher minute ventilation Developmental considerations: Inability to flee dangerous situations, possible increased risk for posttraumatic stress disorder (PTSD) Some vaccines not licensed for children: Anthrax (18 to 65 years), plague (18 to 61 years) Vaccines more dangerous in children: Smallpox, yellow fever Antibiotics less familiar to pediatricians: Tetracyclines, fluoroquinolones

Cardiology Case Study: Acute Pericarditis following Heart Transplantation Patient: A 35-year-old male History: The patient underwent a successful heart transplantation surgery 2 weeks prior. On post-operative day 14, he presented to the hospital with a sudden onset of chest pain and shortness of breath. Examination: Physical examination revealed a fever (101°F) and tenderness over the precordium. An electrocardiogram (ECG) showed evidence of pericarditis with diffuse ST elevations. Diagnosis: The patient was diagnosed with acute pericarditis based on the physical examination findings and ECG changes. Investigations: Blood tests showed elevated white blood cell count and C-reactive protein levels, suggesting an infectious cause. A pericardial fluid analysis revealed a high number of white blood cells, further supporting an inflammatory process. Treatment: The patient was started on nonsteroidal anti-inflammatory drugs (NSAIDs) and colchicine for pain control and to reduce inflammation. He was also started on a course of antibiotics to treat the suspected infectious cause. Outcome: The patient's symptoms improved significantly over the next few days, and he was discharged from the hospital after 6 days of treatment. He was followed up in the clinic regularly, and no further episodes of pericarditis were observed. Discussion: Acute pericarditis is a rare but potentially serious complication of heart transplantation. Prompt recognition and treatment are crucial to prevent complications such as cardiac tamponade, which can be life-threatening. In this case, the patient responded well to a combination of NSAIDs, colchicine, and antibiotics.

Migraine is a primary headache disorder characterized by recurrent moderate to severe headaches, often on one side of the head. It is often accompanied by symptoms such as nausea, vomiting, photophobia, and phonophobia. The pathophysiology of migraine is not fully understood but it is thought to involve the activation of the trigeminovascular system and the release of inflammatory mediators. Migraine triggers include hormonal changes, stress, diet and environmental factors. Diagnosis is based on the patient's history and physical examination. Treatment options include abortive therapy with triptans and non-steroidal anti-inflammatory drugs, and prophylactic therapy with anticonvulsants, beta blockers, and anti-depressants. Non-pharmacological approaches such as cognitive behavioral therapy and biofeedback may also be beneficial.

Sialolithiasis (also termed salivary calculi or salivary stones), is a condition where a calcified mass or sialolith forms within a salivary gland usually in the duct of the submandibular gland. Some factors such as abnormalities in calcium metabolism, dehydration, reduced salivary flow rate, altered acidity (pH) of saliva caused by oropharyngeal infections are involved.
