Cervical cytology screening #Gyni

The indications for surgery in GERD include:- 1- incomplete symptom control with medical management 2- intolerance of, or unwilling nessto comply with, long-term medical therapy 3- regurgitation despite medication 4- presence of a large hiatus hernia 5- complications arising from GORD 6- extraoesophageal symptoms.

Grading of oesophagitis according to the Los Angeles classifcation:- Grade A, one (or more) mucosal breaks no longer than 5 mm that do not extend between the tops of the two mucosal folds (a). Grade B, one (or more) mucosal breaks longer than 5 mm that do not extend between the tops of two mucosal folds (b). Grade C, mucosal breaks that are continuous between the tops of two or more mucosal folds but that involve less than 75% of the circumference (c). Grade D, mucosal break that involves at least 75% of the oesophageal circumference (d).

An oesophageal exposure time of more than 4% can be considered abnormal, and one more than 6% is considered diagnostic of GERD.

prophylactic cholecystectomy for asymptomatic cholelithiasis in the following situations: 1- large (>3 cm) gallstones; 2- choledocholithiasis; 3- chronic haemolytic conditions (sickle cell disease, hereditary spherocytosis); 4- gallbladder polyps >1 cm in diameter; 5- suspicion/risk of malignancy (anomalous pancreatic ductal drainage); 6- calcifcation of the wall (porcelain gallbladder) 7- transplant patients (during transplantation); 8- bariatric surgery. 9- some ethnic groups or subjects living in areas with a high prevalence of gallbladder cancer associated with gallstones(some parts of northern India, Native Americans, Mexican Americans, Colombia, Chile, Bolivia)

◇ Criteria for damage control surgery (DCS) :- 1- Hypothermia <34°C 2- Acidosis: pH <7.2 3- Serum lactate >5 mmol/L 4- Coagulopathy 5- Blood pressure <70 mmhg 6- Transfusion approaching 15 units 7- Injury severity score >36
