📌 ASH 2026 |
مش لازم تستنى الـ Hb يقل عشان تشخّص Iron Deficiency!
من أهم التحديثات الجديدة اللي نزلت خلال هذه اليومين من الـ American Society of Hematology إننا نعيد التفكير في طريقة تفسير الـ Ferritin.
لأن المريض ممكن يكون عنده Iron Deficiency من غير Anemia واضحة.
يعني الـ Hb لسه طبيعي لكن مخزون الحديد قليل بالفعل.
🔹 إيش الأرقام المهمة؟
الـ Ferritin ≤30 ng/mL في البالغين والـ Menstruating individuals يدعم تشخيص Iron Deficiency.
وفي بعض الفئات عالية الخطورة ممكن نستخدم Threshold أعلى يصل لـ ≤50 ng/mL حسب الـ Clinical context.
أما في البالغين اللي عندهم Inflammation فالتفسير بيختلف.
ممكن نفكر في Iron Deficiency عند:
Ferritin <100 ng/mL or TSAT <20%.
وهنا مهم جدا نقيس الـ Ferritin والـ TSAT مع بعض لأن الـ Ferritin ممكن يرتفع بسبب الالتهاب حتى لو الحديد المتاح للأنسجة قليل.
📌 مثال عملي
مريضة عندها Fatigue وHeavy menstrual bleeding
Hb = 12.5
Ferritin = 28 ng/mL
الـ Hb طبيعي لكن الـ Ferritin منخفض وفقا للـ ASH 2026.
فمينفعش نستبعد Iron Deficiency لمجرد إن الهيموجلوبين طبيعي.
وفي نفس الوقت لازم ندور على السبب زي فقد الدم أو سوء التغذية أو Malabsorption.
✨الخلاصة ان مش كل Iron Deficiency لازم تكون مصحوبة بـ Anemia.
ومش كل Ferritin طبيعي يستبعد نقص الحديد خصوصا مع Inflammation.
لكن أيضاً مش كل مريض تاعب محتاج Iron.
التشخيص الصح بيبدأ من فهم الـ Clinical context وتفسير الـ Iron studies بشكل سليم.

ASH 2026 provides diagnostic ferritin thresholds, not treatment targets.
A commonly used practical repletion endpoint in the literature is ferritin ≥50 ng/mL after correction of iron deficiency, when inflammation is absent.
This ≥50 ng/mL level is not an ASH treatment target.
It should be considered a practical, non-ASH reference point
Disease-specific Serum Ferritin targets differ in conditions such as:
Chronic kidney disease
Heart failure
Inflammatory bowel disease
Pregnancy
Chronic inflammatory states

حالتان في نفس اليوم لعضات كلاب، وكلتاهما لأطفال!
كطبيب، من المؤسف أن نشاهد مثل هذه الحالات بشكل متكرر.
الكلاب الشاردة أصبحت خطراً حقيقي على أطفالنا، وليست مجرد ظاهرة مزعجة.
عضة واحدة قد تؤدي إلى عدوى والتهاب، وتهتك في الأنسجة والعضلات، وقد تصل مضاعفاتها إلى أخطر من ذلك، مثل الكزاز أو السعار.
حماية الأطفال لا تكون بعد وقوع العضة فقط، بل تبدأ من الحد من انتشار الكلاب الشاردة والتعامل مع هذه الظاهرة بجدية من الجهات المختصة والمجتمع.

💊 Azithromycin:
• Adult dose: 1 g (جرعة واحدة)
• Pediatric dose: 20 mg/kg (جرعة واحدة)
بالنسبه للاطفال هتكون الجرعه وزن الطفل مره واحده يوميا من تركيز azithromycin 200
بشرط ان لا يزيد اجمالي الجرعه للطفل عن جرعه ال adult
مثال طفل وزنه 10 كيلو ياخد جرعه واحده ١٠ مل بالسرنجه من تركيز azithromycin تركيز 200 مثل xithrone 200 mg susp
مره واحده لا تكرر

💊 Doxycycline:
• Adult dose: 300 mg (جرعة واحدة)
• Pediatric dose: 4-6 mg/kg (جرعة واحدة)

أحدث البروتوكولات بالجرعات والمدد
🔴 لماذا لا نوصي ببروتوكولات Clarithromycin؟
أظهرت الدراسات الحديثة تزايد معدلات مقاومة H. Pylori لمضاد Clarithromycin، مما أدى إلى انخفاض نسب نجاح العلاج المرتكز عليه. لذلك، تُوصي الإرشادات الحالية باستخدام بروتوكولات بديلة أكثر فاعلية للقضاء على العدوى.
💊 أحدث البروتوكولات العلاجية:
1️⃣ Dual Therapy (العلاج الثنائي):
Vonoprazan: 20 مجم مرتين يوميًا.
Amoxicillin: 1 جرام ثلاث مرات يوميًا.
المدة: 14 يومًا.
2️⃣ Bismuth Quadruple Therapy (العلاج الرباعي المحتوي على البزموت):
Bismuth Subsalicylate: 120-300 مجم أربع مرات يوميًا.
Tetracycline: 500 مجم أربع مرات يوميًا.
Metronidazole: 250-500 مجم ثلاث أو أربع مرات يوميًا.
Proton Pump Inhibitor (PPI): مثل Omeprazole 20 مجم مرتين يوميًا.
المدة: 10-14 يومًا.
3️⃣ Rifabutin Triple Therapy (العلاج الثلاثي بالريفابيوتين):
Rifabutin: 150 مجم مرة يوميًا.
Amoxicillin: 1 جرام مرتين يوميًا.
PPI: مثل Omeprazole 20-40 مجم مرتين يوميًا.
المدة: 10-14 يومًا.
📌 ملاحظات هامة:
اختبار المتابعة:
بعد انتهاء العلاج، يُوصى بإجراء اختبار مثل Urea Breath Test أو Stool Antigen Test للتأكد من القضاء على العدوى.
اختيار البروتوكول:
يعتمد على حالة المريض، والتاريخ العلاجي، ومقاومة المضادات الحيوية.
أفضلية العلاجات الحديثة: البروتوكولات غير المرتكزة على Clarithromycin تُظهر نسب شفاء أعلى وتقلل من خطر المقاومة البكتيرية.

🩺 | Bell's Palsy ( Facial Nerve Palsy)
بيانات الحالة: مريضة في الثلاثينات من العمر، لا تعاني من أي أمراض مزمنة أو تاريخ مرضي يُذكر.
📍 الشكوى الرئيسية: عدم القدرة على إغلاق العين اليمنى مع انحراف في زاوية الفم إلى الجهة اليسرى منذ الصباح.
🩺 الفحص السريري:
- العلامات الحيوية مستقرة.
- الفحص العصبي سليم، ولا توجد أي علامات توحي بسكتة دماغية أو إصابة عصبية مركزية.
- لا توجد فقاعات أو طفح في الأذن.
❇️ التشخيص : Bell's Palsy
🚨 Red Flags
- Forehead sparing
- Limb weakness and aphasia
- Altered consciousness
- Cerebellar signs
- Recurrent facial palsy
- Bilateral facial weakness
- Progressive symptoms
- Additional cranial nerve involvement.
❇️ الخطة العلاجية:
Rx – Bell's Palsy (Facial palsy)
1. Prednisone 20 mg tab
Take 1 tablet PO TID after meals × 5 days.
Then:
Prednisone 10 mg tab
Take 1 tablet PO TID after meals × 5 days.
*Monitor blood pressure and blood glucose.*
2. Valacyclovir 1 g tab
Take 1 tablet PO TID × 7 days.
OR
Acyclovir 400 mg tab
Take 1 tablet PO five times daily × 10 days.
3. Artificial Tears
Instill 1–2 drops into the affected eye every 2–4 hours while awake.
+ Lubricating eye ointment at bedtime.
+ Eye patch/taping during sleep if unable to close the eye completely.
4. Pantoprazole 20–40 mg
Take 1 tablet PO once daily, 30 minutes before breakfast × 10 days.
📚 نقاط تعليمية مهمة:
🔹 Bell's palsy accounts for approximately 60–75% of all cases of unilateral facial paralysis
🔹 Forehead involved = Think LMNL (Bell's palsy).
🔹 Forehead spared = Think UMNL (Stroke) until proven otherwise.
🔹 Facial palsy present with any one of red flag should evaluate for stroke and warrants Brain CT to exclude a central lesion as Stroke.
🔹 Corticosteroids are the only treatment with strong evidence for improving complete recovery and should be started within 72 hours of symptom onset.
🔹 Antiviral therapy may provide additional benefit in severe paralysis, but should never replace corticosteroids.
🔹 Eye protection is mandatory in patients with incomplete eyelid closure to prevent exposure keratitis and corneal ulceration.
🔹 Baseline BP and RBS should be assessed whenever feasible before initiating corticosteroids therapy, particularly in patients with known hypertension, diabetes mellitus, or other risk factors. Both should be monitored during treatment.

A 55-year-old male from Amran, with unknown case of any chronic medical illness, presented with chronic throbbing headache for 1.5 months, associated with bilateral tinnitus and palpitations.
He denied any bleeding tendency from any body orifice, including hematemesis, melena, hematochezia, hematuria, or epistaxis. There was no history of purpura, petechiae, or recurrent infections.
-Constitutional symptoms : There's decrease activity, No fever no wight Loos, no sweating,

Chronic watery diarrhea for the past 4 months.
The remainder of the systemic review was irrelevant

O/E
The patient looks ill, cachectic with Marked severe pallor was noted (as shown in the photo), and Bilateral painless pitting lower limb edema, extending up to the mid-leg.
No jaundice, No Cyanosis, No palpable lymphadenopathy.

Atropine IV: 2 mg, double every 5–10 min → till atropinisation (dry chest, HR > 80)
Pralidoxime IV: 1–2 g over 30 min → infusion 200–500 mg/hr

Clinical Case
❇️ Patient Information
Age: 2 years, his wight 10kg
Sex: Male
Residence: Amran, Yemen
❇️ chief complaint
Bilateral periorbital swelling (eye puffiness) associated with Dark-colored urine for 5 days.
❇️ History of Present Illness
A 2-year-old boy presented to the Emergency Department with bilateral periorbital swelling associated with dark-colored urine for 5 days.
According to his mother, the child had developed a sore throat approximately two weeks before presentation, which resolved spontaneously without medical evaluation or antibiotic treatment.
Five days prior to admission, she noticed the gradual onset of bilateral eyelid swelling, which was more prominent in the early morning upon waking and later became persistent throughout the day. Around the same time, his urine became dark-colored (tea-colored), and he developed a high-grade fever.
There was no obvious decrease in urine output, and no dysuria, urinary frequency, or loin pain. No history of generalized body swelling was reported.

Other systems: irrelevant
☄ Past Medical History
Malaria 6 months earlier requiring blood transfusion and antimalarial treatment.
☄ Physical Examination
The child was conscious, alert and responsive, with stable general condition.
Vital Signs
- Temperature 39°C
- Blood Pressure 150/120 mmHg
- Pulse 108 bpm
- Respiratory Rate 33/min
- SpO₂ 95% on room air
Systemic Examination
- General: Bilateral periorbital edema and Mild pallor.
- Chest : Clear with Good bilateral air entry
- CVS: S1S2+0, No murmurs
- Abdomen: Soft, Non-tender, No hepatosplenomegaly.
- CNS: Conscious and alert, No focal neurological deficits.
⚠️ Initial Differential Diagnosis

1. Acute Post-Streptococcal Glomerulonephritis (most likely)
2. Malaria
3. IgA nephropathy
4. Nephrotic syndrome
طلبنا للطفل الفحوصات التالية

- ASO
- RBS
- MPS for malaria
- Urinalysis
- serum Creatinine
- Serum Albumin , ALT
- Abdominopelvic Ultrasound
طلعت النتائج الآتية:

Hb 7.7 g/dL
MCV 63.5 fL
MCH 17.1 pg
MCHC 26.9 g/dL
❇️ Interpretation
Moderate microcytic hypochromic anemia
➡️ Inflammatory Markers
- CRP Positive (++++)
- ASO titre Positive (++++)
❇️ Interpretation
Strong evidence of a recent Group A Streptococcal infection.
➡️ Biochemistry
- Random Blood Sugar 80 mg/dL
- Creatinine 0.4 mg/dL
- Serum Albumin 4.0 g/dL
- ALT (SGPT) 16 U/L
❇️ Interpretation
Normal renal function and normal serum albumin.
➡️ Urinalysis:
Macroscopic: Tea-colored, turbid urine.
Microscopic: RBCs: 8–10/HPF, Protein: (+)
❇️ Interpretation: Microscopic hematuria.
➡️ Malaria Parasite: Negative.
➡️ Abdominal Ultrasound:
- Mild hepatomegaly.
- Few enlarged mesenteric lymph nodes.
- Otherwise unremarkable abdominal ultrasound.
- Final Clinical Impression
❇️ Probable Acute Post-Streptococcal Glomerulonephritis (APSGN)
Supported by
- Recent streptococcal sore throat (2 weeks earlier).
- Bilateral periorbital edema.
- Dark-colored urine.
- Strongly positive ASO titre and Elevated CRP.
- Severe hypertension.
- Normal serum albumin.
❇️ And Moderate microcytic hypochromic anemia for further evaluation.
⚠️ Management During ED
❇️ Supportive Care
- Medications
1⃣ Furosemide (Lasix) 1–2 mg/kg IV stat
2⃣ IV Paracetamol (Perfalgan) 15 mg/kg PRN for fever
3⃣ Ceftriaxone 500 mg IV twice daily AST
4⃣ Crystalline Penicillin G 50,000 IU/kg/dose IV q6h (AST)
❇️ Advice and further evaluation in Pediatric Ward:
- Strict blood pressure monitoring.
- Strict input/output chart & Daily weight monitoring
- Fluid and salt restriction
- 24-hour urine collection.
- Pediatric follow-up.
- Send for Blood film morphology, Sickling Test, Reticulocyte count.
- send for Serum C3 complement
- Check Serum electrolyte.

📍وصلت إلى قسم الطوارئ امرأة تبلغ من العمر 30 عام من محافظة عمران، تشكو من حمى مستمرة منذ ثمانية أيام.
ذكرت أن الحمى بدأت تدريجياً، ثم أصبحت مستمرة طوال اليوم، ولا تنخفض إلا بشكل بسيط بعد تناول خافضات الحرارة، ثم تعود للارتفاع مجدداً.
📝 History of Present Illness
عند أخذ التاريخ المرضي بشكل أوسع، أوضحت أنها تعاني أيضًا من:
- تعب وإرهاق شديد مع خمول عام
- صداع مستمر
- غثيان مع فقدان للشهية
- ألم خفيف منتشر بالبطن
Review of Systems
عند مراجعة أجهزة الجسم، لم تذكر:
- كحة، ضيق تنفس، أو ألم بالصدر.
- طرش او ترجيع دم، إسهال أو إمساك، اصفرار الجلد أو العينين، أو انتفاخ البطن.
- لا توجد اي أعراض بولية
- لايوجد طفح جلدي أو آلام أو تورم بالمفاصل.
📝 Past History
نفت وجود: أي أمراض مزمنة أو عمليات جراحية سابقة او استخدام أدوية بشكل دائم، لم تسافر خلال الأشهر الثلاثة الماضية، ولا يوجد حالات مشابهة في العائلة
📝Physical Examination
كانت المريضة تبدو مرهقة قليلاً، لكنها واعية ومتعاونة وتتحدث بصورة طبيعية.
Vital Signs
- Temperature: 39.3°C
- Blood Pressure: 110/70 mmHg
- Pulse: 82 bpm (Relative bradycardia)
- Respiratory Rate: 18/min
- Oxygen Saturation: 98% on room air
❇️ Systemic Examination
- Chest: clear with good air entry bilateral
- Cardiovascular: S1S2+0,No murmurs.
- Abdomen: Soft, No guarding or rigidity, No hepatosplenomegaly.
- CNS: Conscious, oriented, No focal neurological deficits.
📌 التشخيصات التفريقية لمريضة تعاني من ⬇️
Fever lasting more than one week associated with constitutional symptoms, relative bradycardia, and no obvious focus of infection.
Ddx:
- Enteric (Typhoid) fever.
- Malaria.
- Dengue fever.
- Acute viral illness.
- Brucellosis.
نظراً لأن الاشتباه السريري كان مرتفع نحو حمى التيفوئيد، طُلبت الفحوصات التالية:
- CBC, CRP, LFT, RBS
- Urinalysis
- Widal test
- MPS
- Abdominal U/S
وتم إدخال المريضة إلى قسم الملاحظة (ED) لحين ظهور النتائج، مع البدء بالعلاج الداعم:
- Normal Saline 0.9% 500ml IV
- Perfalgan (Paracetamol) 1 g IV.
- Ondansetron 4 mg IV.
- Pantoprazole 40 mg IV.
- Ceftriaxone (Forcef) 1 g IV stat dose
نتائج الفحوصات
- CBC
Hb: 13.4 g/dL
WBC: 3,200/µL
Platelets: 170,000/µL
- CRP: 68 mg/L
- LFT: Normal.
- RBS: Normal.
- Urinalysis: Normal.
- Widal test: Positive.
- MPS: Negative for malarial parasites.
- Abdominal ultrasound: Normal.
☆ Blood culture could not be performed because of resource limitations.
✅ Final Clinical Impression
Probable Enteric Fever (Typhoid Fever) based on:
- Fever lasting more than one week
- (Constitutional symptoms).
- absence focus of infection
- Relative bradycardia.
- Leukopenia.
- Positive widal test كعامل داعم، مع الأخذ في الاعتبار محدودية دقة الفحص
وبسبب عدم توفر مزرعة الدم، والتي تُعد المعيار الذهبي للتشخيص، اعتمد التشخيص على الصورة السريرية والنتائج المخبرية الداعمة.
📗 Treatment on Discharge
📍 Azithromycin 500 mg PO once daily for 7 days

- اضطراب في الوعي.
- استمرار الحمى بعد خمسة أيام رغم العلاج.
💎 Clinical Pearls
✅ Blood culture remains the gold standard for diagnosing typhoid fever, particularly during the first week of illness and before initiating antibiotics.
✅ In endemic areas, malaria should always be excluded in any patient presenting with prolonged fever, regardless of the initial clinical suspicion.
✅ Relative bradycardia (Faget sign) is a classic clinical clue that may strongly suggest enteric fever, although it is not specific.
✅ Leukopenia associated with prolonged fever supports the diagnosis of typhoid fever.
✅ The Widal test should never be used alone to establish the diagnosis because of its limited sensitivity and specificity, especially in endemic areas.
✅ The most serious complications are intestinal hemorrhage and ileal perforation, which usually occur during the third week if treatment is delayed.
