Clinical case :راجل عنده 38 سنة، جاله مغص شديد في البطن مع انتفاخ وقيء، ومبقاش بيخرج stool أو gases من يومين. عنده history إنه عمل abdominal surgery من سنة.
في الفحص:abdominal distension high-pitched bowel sounds X-ray abdomen أظهرت multiple air-fluid levels. ايه التشخيص ؟

يا أخي الإنسان ده!
يبقى دخل ١٠٠ امتحان في الكلية، وربنا كرمه في كل واحد، بأكتر من اللي يستحقه، وما زال لا يُحسن الظن بالله!
-دخلت وأنت مريض، وسترها معاك؛ عشان يعلمك إنه بيجازيك حتى على محاولة المذاكرة وأنت عيان في السرير.
-ودخلت وأنت مش مذاكر وسترك؛ عشان يعرفك إن النجاح عمره ما كان من عندك؛ بل من عنده.
-دخلت وكان الامتحان صعب وجبت درجة أعلى من اللي تخيلتها.
-هداك تذاكر الفايل اللي جه منه أغلب الامتحان.
-هداك تقرأ على باب اللجنة الجزئية اللي هتدخل تلاقيها.
-ساعدك تفتكر السطر اللي فيه الإجابة.
وده كله ولسه بتخاف لما بس حاجة في خطتك تبوظ، وهي عمرها ما مشيت بخطتك أساسًا!

Clinical Case :ست عندها 36 سنة، جاية الطوارئ بسبب: ألم شديد في البطن إسهال مدمم حرارة وإرهاق شديد الأعراض بدأت من حوالي أسبوعين، لكن من يومين الألم زاد جدًا. هي عندها history of ulcerative colitis من سنين، وكانت ماشية على العلاج لكن وقفته من نفسها من شهر.
في الفحص:الحرارة 39°C Pulse: 125/min BP: 90/60 البطن منتفخة جدًا diffuse abdominal tenderness bowel sounds شبه غايبة
التحاليل:Hb منخفض ESR و CRP عاليين leukocytosis شديد X-ray abdomen أظهر: dilated colon أكثر من 6 cm ايه هو التشخيص المحتمل هنا ؟ وايه أخطر complication متوقع لو الحالة اتأخرت؟

في الازمات اللى موجوده في اى دوله دلوقت مين الجندى المجهول اللى محدش بيجيب سيرته ولا غنى عنه ولو كل الناس استخبوا هو اللى بيروح شغله ومينفعش يقعد أو يسرحوه من شغله !! هو اللى بيشتغل في الطب ، صحة الإنسان وحياته .. مفيش حد يعرف يستبدله ، ولا يحل محله اى مهنة تانيه ،
انتوا بتعملوا عظمه بجد ، لازم تكونوا شاطرين ومتفوقين ، الله يبارك في دراستكم وعملكم وينفع بيكم .
