في طريقة لحفظ ال cytokines بتاعت المخطط: ✅ _بالنسبة للخط بتاع الlymphoid_ : - فيه ألفا وبيتا :(TNF alpha - TGF beta) - فيه حاجات مجموعها 12: IL2 + IL7 + FLT3 = IL12 ومن *الصدف* إن الاسكيمة أرقامها مترتبة (SDF-1) بعد كدة انزل لإنتاج خلايا الT: عندك الارقام الزوجية: IL2-4-6 وهتنقص واحد من الأول (IL1) وتزود واحد على الأخير (IL7) _✅ بالنسبة للخط بتاع الMyeloid:_ - الIL3 والGM-CSF موجودين في كله بس خلي بالك في الmonocyte هتزود M-CSF وفي الneutro&baso هتزود G-CSF - الIL6 والSCF موجود في كل نواتج ال myeloblast ماعدا الEisinophil بتاخد IL5 ومفيهاش SCF - الEPO مع خط الRBCs والTPO مع خط ال platelets ودي سهلة من أسماءهم

⚠️ Radiotherapy: ✍️Classification of radiotherapy. 1️⃣ According to aim: a- curative . بستخدمها لما ببقى مش قادر امد ايدي في العيان بجراحة زي الحاجات دي: used mainly for inoperable cases as brainstem tumors, irresectable tumors , patient unfit for surgery و بعدها المفروض العيان يكون اتعالج تماما b- palliative: to decrease suffering as in metastatic tumors to bone, brain c- prophylactic : as in acute lymphoblastic leukemia : this tumor lead to metastatis to brain commonly, which may be site of recurrence after treatment فبتدي اشعاع على المخ كله علشان تحميه من كدة . 2️⃣ Classification according to timing: a- neoadjuvant= preoperative radiotherapy : to decrease size of tumor as in colorectal cancer b- chemoradiotherapy = concurrent radiotherapy بدي الاتنين سوا as in lung , brain cancer c- adjuvant radiotherapy= postoperative radiotherapy as in breast cancer. ✍️ Types of radiotherapy: 1- cobalt 60 جهاز قديم و مش بيحبوا يستعملوه لان اعراضه الجانبية كتير و فعاليته اقل من باقي الحاجات. 2- linear accelerator و ده اكتر جهاز بيستخدموه 3- proton therapy: حلو بس مش في مصر 4- electeone beam therapy: يفرق عن الفوتون في انه قوته على الاختراق ضعيفة فبالتالي Only useful in treatment of superficial cancers as skin cancer, parotid gland cancer due to low penetration. But NOT useful in deep cancers. و ده يفرقها عن الفوتون اللي قدرتها على الاختراق بتكون أعلى 5- brachytherapy: اشعة داخلية.   بدخلها جوة و هي تبدأ تطلع إشعاع تدمر الورم. Seeds Used in cancer cervix stage 1A1 ( early stage) and in prostate cancer ✍️radiation toxicity: 1- acute toxicity: within 2-3 weaks Examples: dermatitis, pneumonitis after breast irradiation.   Pneumonitis تحصل بالذات لو العيانة كانت الرئة بتاعها فيها مشكلة زي مثلا copd + Mucositis as in esophagus leading to severe dysphagia as in neck radiation used in larngeal , hypopharyngeal cancer. + Proctitis, diahrea, cystitis in pelvic irradiation. 2- subacute toxicity 3- late toxicity: تحصل لو العيان كان اصلا عنده مشكلة في الكبد زي cirrosis مثلا . فبعد  ما عالجت سيبت كبد بايظ   🌿Other systemic therapies for cancer: ⚠️ Chemoradiotherapy - action : induce cell apoptosis - effective mainly in rapidly dividing cells as in leukemia and lymphoma ( MCQ) ✍️ Indications : 1- Neoadjuvant= preoperative:  as in triple negative breast cancer 2- adjuvant= post operative 3- chemoradiotherapy= concurrent ⚠️Hormonal treatment : - used in prostate cancer in males, breast cancer in female - by cicatrization either *Surgical: removal of ovary in female, testes in males, however in didn't deal with sex hormones production from others cells as suprarenal gland. * Irradiation * Medical: In female: Before menopause: tamoxifin After menopause: aromates inhibitors In males : antiandrogen as GnRH agonist.. - side effects: as menopause in females الراجل و الست بيبقوا نفس الأعراض الجانبية تقريبا. ✍️ Receptors of breast cancer: ER, PR , Hair-2 ( receptor for target therapy) If the three receptors are negative: triple negative breast cancer as in BRCA mutation. ⚠️ Immunotherapy: - idea: bind to PD-L1 receptor و لما يمسك فيه زي ما يكون كشف الورم قدام المناعة فالمناعة تدمر الورم. - used in : 1-Melanoma: most sensitive tumor to Immunotherapy 2- triple negative breast cancer مشاكله: - never give steroid before giving Immunotherapy: مش هيستفاد منه - can lead to widespread inflammation all over the body due to hyperactive immune system. In this case inflammation may be controlled by steroids. - contradicted in hepatitis C , Hashimoto thyroditis or patient suffering from any autoimmune disease. - side effects: rash, diahrea, fatigue, inflammation. ⚠️Target therapy: -Bind to specific receptor in the tumor - examples: cituximab : monoclonal antibody احفظوا الاسم بتاعه مهم في الامسيكيو. ممكن اديه: Compined with radiotherapy and Immunotherapy either: -Concurrent: كله مع بعض - sequential: ده ورا ده . ⚠️Oncolgical emergency: اهم الاتنين: 1- spinal cord compression Treatment: radiotherapy within 48 h to avoid irreversible paralysis 2- bleeding
