"المريض على Noradrenaline… إمتى نزود وإمتى نقلل؟ ⚠️💉"

أولًا: هو إيه Noradrenaline بيعمل؟
Norepinephrine
هو Catecholamine بيشتغل أساسًا على:
α1 receptors → vasoconstriction ↑
β1 receptors → دعم بسيط للقلب
النتيجة:
↑ systemic vascular resistance (SVR)
↑ blood pressure
تحسين perfusion للـ vital organs
إحنا بنستهدف إيه في الـ ICU؟
الهدف مش “ضغط عالي”… الهدف هو:
➡️ Mean Arterial Pressure (MAP) ≥ 65 mmHg
عشان نضمن perfusion للكلى، المخ، والقلب
لكن الرقم لوحده مش كفاية.
إمتى تزود الجرعة؟ (Titration up)
نزود تدريجيًا لو:
1) MAP أقل من الهدف
< 65 mmHg بشكل مستمر
2) علامات ضعف perfusion:
urine output ↓ (<0.5 ml/kg/hr)
lactate ↑ (↑ anaerobic metabolism)
altered mental status
cold clammy extremities
👉 هنا المشكلة مش رقم ضغط بس… دي shock still ongoing
إمتى نقلل الجرعة؟ (Titration down)
نقلل تدريجيًا لو:
1) MAP أعلى من الهدف بشكل مستمر
70–75 mmHg بدون داعي
2) علامات vasoconstriction زيادة:
أطراف باردة جدًا
ضعف peripheral perfusion
possible digital ischemia (في الحالات الشديدة)
👉 هنا إحنا ممكن نكون over-supporting
مهم جدًا: مش كل ضغط = حالة مستقرة
ممكن تلاقي:
MAP طبيعي
لكن:
lactate عالي
المريض لسه في shock
👉 ده اسمه compensated numbers but poor perfusion
إزاي نقيّم صح في ICU؟
ما تعتمدش على الضغط بس… لازم تشوف:
✔️ MAP
✔️ urine output
✔️ lactate
✔️ mental status
✔️ capillary refill / skin perfusion
أخطاء شائعة في الـ ICU:
🚫 رفع الجرعة بسرعة بدون إعادة تقييم
🚫 الاعتماد على MAP فقط
🚫 تجاهل lactate trend
🚫 نسيان إن السبب الأساسي للـ shock لسه موجود (sepsis / hypovolemia)
الخلاصة:
Noradrenaline مش علاج…
ده support مؤقت لحد ما تعالج السبب الأساسي للـ shock

“المعيار الحقيقي مش الضغط… المعيار هو التروية (Perfusion)”
