Pseudo exfoliation Glaucoma (PXG)
A secondary open-angle glaucoma caused by clogging
of the trabecular meshwork with pseudoexfoliative
material.
More aggressive than primary open-angle glaucoma
(POAG), with higher IOP fluctuations and faster
progression.
Poor response to medications, often requiring laser
trabeculoplasty (SLT) or surgery (trabeculectomy,
glaucoma drainage devices).
Slit-lamp examination:
Look for white deposits on the anterior lens
capsule (central disc with a clear peripheral
zone).
Check for iris transillumination
defects(peripheral moth-eaten appearance).
Assess corneal endothelium (may show
"Krukenberg spindle"-like deposits).
Gonioscopy:
Pseudoexfoliative material in the trabecular
meshwork (TM) (often patchy pigment
deposition).
IOP Measurement:
Often asymmetric (higher in the eye with more
PEX deposits).
Diurnal fluctuations are common.
