Hypertensive crisis mx :- العلاجات الي نستخدمها بالطوارئ لعلاج ارتفاع الضغط منكدر نستخدمها بجميع المرضى ولازم تخلون هذا الشي بالحسبان من اريد اختار العلاج الصحيح ، هاي الاشياء تعتبر Contraindications لكل علاج ومتخليني اختاره ، وكالاتي :- 1-Labetelol :-

Asthma , bradycardia , cardiogenic shock, decompensated HF.

2-Nicardibin :-

severe aortic stenosis.

3-ACE/ARB :-

pregnancy , bilateral renal artery stenosis , hyperkalemia.

4-sodium nitroprusside :-

renal and hepatic

dysfunction , high doses should be avoided due to cyanide toxicity , ICP.

5	مساء الخير ، شوية ملاحظة مهمة نركز بيها : كلنا نعرف ال BNP الي نستخدمه ك investigation in heart failure , بس عدنا كم شغلة ضروري جدًا جدًا نعرفها . بالمايوسايت اكو فد شي اسمه Pro BNP من يصير اكو فد لود على ال ventricles هذا راح يطلع ويتكسر إلى شغلتين Pro BNP -> BNP + NT Pro BNP شنو الفرق بينهن ؟ ال BNP الهالف لايف مالته ٢٠ دقيقة والجسم مالتنا يتخلص من عنده عن طريق إنزيم اسمه Neprilysin هذا يكسر ال BNP . بينما الNT pro BNP جسمنه يتخلص من عنده عن طريق ال kidneys فتكون الهالف لايف مالته اكثر من ٦٠ إلى ١٢٠ دقيقة. اكو شغلة مهمة : كلنا سامعين بعلاج ال sacubitril/valsartan هذا العلاج شنو يسوي؟ يثبط إنزيم ال neprilysin بالتالي ال BNP ما راح يتكسر فيبقى عالي لذلك اي مريض ماشي على هذا العلاج ما يصير نسوي فحص ال BNP انما نسوي NT pro BNP لان ماله علاقة بالعلاج . بنفس الوقت جماعة ال renal failure منكدر نسويلهم NT pro BNP لان راح يكون عندهم عالي وميفيدنا ملاحظة اخيرة :- ذني إذا صعدن المريض ممكن يكون عنده HF بس مو اكيد ، بس اذا شفناهم نازلين فكلش يفيدون نستثني الHF يعني Negative predictive value	2 591
6	🩺 OSCE — Peripheral neuropathy in a diabetic patient من أهم الأسئلة اللي ممكن ترد بالـ OSCE: What are the important causes of peripheral neuropathy in a patient with diabetes mellitus? الجواب عندنا سببين مهمين لازم نفكر بيهم: 1️⃣ Diabetic peripheral neuropathy — وهو الأكثر شيوعًا. 2️⃣ Vitamin B12 deficiency — وخصوصًا إذا المريض يستخدم metformin لفترة طويلة. ⸻ ❓ زين، ليش السبب الأول هو Diabetic peripheral neuropathy؟ لأن الـ chronic hyperglycemia يسبب metabolic and microvascular damage to the peripheral nerves، فيؤدي إلى distal symmetric polyneuropathy. يعني المريض غالبًا يجي عنده burning pain, tingling, numbness بالقدمين، ومع الفحص نلاحظ reduced ankle reflexes وsensory loss. وغالبًا يبدأ من القدمين بشكل stocking distribution، وإذا تقدم المرض ممكن يوصل لليدين ويسوي glove distribution. 🔴 مهم جدًا: الـ diabetic neuropathy نفسها ممكن تسبب loss of vibration وloss of proprioception، لذلك لا تعتبرهم وحدهم دليل على B12 deficiency. ⸻ ❓ زين، ليش نفكر بـ Vitamin B12 deficiency؟ لأن الـ B12 deficiency مو بس تأثر على peripheral nerves، وإنما ممكن تأثر أيضًا على posterior columns and corticospinal tracts بالـ spinal cord. فممكن نشوف loss of vibration and proprioception مع sensory ataxia، وإذا صار corticospinal tract involvement ممكن تظهر UMN signs مثل hyperreflexia, increased tone and Babinski sign. ولهذا B12 deficiency تعتبر important and treatable alternative cause للـ neuropathy عند مريض السكري.	2 227

10	أسهل طريقة حتى نتذكر الـMurmurs ببساطة، بالبداية لازم نعرف أسماء الـvalves من اليمين لليسار: Tricuspid → Pulmonary → Aortic → Mitral ونختصرهن بأول حرف: T → P → A → M 🔴 هسة شنو يصير بالـSystole؟ خلال الـsystole، لازم الـAortic والـPulmonary valves ينفتحن. فإذا ما انفتحن زين، يعني صار بيهن Stenosis، راح يطلع عدنا murmur. لذلك نحط حرف S تحت: P → Pulmonary stenosis A → Aortic stenosis بنفس الوقت، خلال الـsystole لازم الـMitral والـTricuspid ينسدن. فإذا بقن مفتوحات وصار بيهن Regurgitation، راح يطلع عدنا murmur. لذلك نحط حرف R تحت: T → Tricuspid regurgitation M → Mitral regurgitation 🧠 يعني بالـSystole نحفظها هيچ: T → R P → S A → S M → R يعني: TR + PS + AS + MR 🔄 هسة نجي للـDiastole الـdiastole يكون بالعكس تمامًا. الـAortic والـPulmonary لازم ينسدّن، فإذا بقن مفتوحات يصير Regurgitation. والـMitral والـTricuspid لازم ينفتحن، فإذا ما انفتحن زين يصير Stenosis. يعني: T → S P → R A → R M → S TS + PR + AR + MS	3 297
