This pt presented with asthma exacerbation (known case of asthma). O/E SPO2 87% Diffuse wheeze Limbs examination revealed vasculitic-like lesions. Drug hx : she said that her condition exacerbated after taking the tab before dinner!!!!! And this rash is self limiting but painful. Suspicion of Churg Struass syndrome was come and the pt referred to rheumatologist after her stabilization. #rheumatology

Plavix regimen in acute coronary syndrome : 1. NSTE-ACS : -- age <75 : give 300mg -- age > 75 : give 75mg 2. STEMI with thrombolytics as a mode of re-perfusion : -- age > 75 : give 75mg -- age < 75 : give 300mg. 3. STEMI with PCI as a mode of re-perfusion : Give 600 mg REGARDLESS the age. Post-alteplase lingual edema, very rare but extremely serious side effect. Mx : 1) 1st line : Benadryl IV 50 + zantac 50 mg 2) if no response to 1st line : Methylpredinsolone 100mg IV 3) if no response give : adrenaline 0.5mg IM or SC 4) If no response, consult ENT specialist #emergency #pharma

📑 The Triangle of Auscultation It is a space on the back where the relatively thin musculature allows for respiratory sounds to be heard more clearly with a stethoscope. 🔖 To make sound better on stethoscope ? Ask patient fold his\her arms across their chest, medially rotating the scapulae, while bending forward at the trunk, to better expose the floor of the triangle.. ———— 📑 The two murmurs that can be heard in the patient’s back: 👉 Mitral regurgitation (MR) 👉 Coarctation of the aorta (COA) 🔖NOTE 👉 Systolic murmur Coarctation - loudest in the back below the left shoulder blade. 👉 MR due to anterior leaflet disease - pansystolic murmur radiates to the axilla and back — as the regurgitant jet is directed posterolaterally.
