📑 Melena It is the passage of black tarry colour and offensive smell, and is often difficult to flush away, which is due to the alteration and degradation of blood by intestinal enzymes as a result of upper gastrointestinal bleeding. 👉 Blood should remain in the gut for 8 - 14 hr approximately. 👉 It usually means bleeding episodes from sites above the ligament of Treitz. 👉 Even up to middle of transverse colon can produce melena. 👉 It takes 60 mL- 100 mL or more of blood in the stomach to turn stools black. 👉 The most common cause of UGIB is gastric -\+ duodenal ulcers 👉 One episode of bleed can produce 5–7 episodes of melena. 🔖NOTE: Bleeding from URT can be mistaken for GI bleeding because the blood can be swallowed and vomited or appear in the stool as melena. Careful examination & Hx taking is the key to differentiating respiratory sources of bleeding from GI ones.. #git

Hidradenitis suppurativa develops in some people after puberty when the hair follicles (the pores in the skin where hair grows) under the arms, in the groin, and around the nipples and anus are chronically blocked and become inflamed. The hair follicles may swell and form bumps that look like acne we do not know the cause of blockage, but it is not related to poor hygiene, to the use of deodorants or powders, or to underarm shaving. The blockage causes the hair follicles to swell and rupture, sometimes leading to infection by various bacteria. The abscesses (pus-filled pockets) that result are tender, painful, and foul smelling, can grow deep into the skin, and tend to reappear if they heal. After the abscesses heal and reappear repeatedly, the skin in the area becomes thick and scarred. Often, permanent channels (called sinus tracts) form between the abscess and the skin surface and drain pus. How we treat it 1) mild cases : intralesional steroids+ washing of area by benzol peroxide + topical and systemic clindamycin 2) moderate cases : drainage of abscess + washing by benzyol peroxide + rifampin for 3 months 3) severe cases : we use infliximab IV #surgery

مريض عنده Pulmonary embolism انطيناه heparin Bolus dose : 10,000 u الساعة ٨ مساءاً Infusion dose : 1500 U/hr نفس اليوم الساعه ب 2 الصبح : صار عنده haemoptysis و كان ال aPTT تقريباً 90second شلون راح تحسب الـ dose مال الـ prtoamine sulfate لهذا المريض . يلا شباب أنطلقوا @AASmedicalbot

STEMI equivalents كلش موضوع مهم، مجموعة من الحالات ألي تكون equal to the STEMI in managment، لكن تختلف بالـ ECG و تختلف بالـ clinical picture (تقريباً) : أكو حالات كمن و الكل يعرفها ألي هن : 1. LBBB with +ve Sgarbossa 2. Wellens syndrome 3. de Winter syndrome 4. Posterior MI 5. Hyper acute T wave أكو حالات rare و قليلين ألي يعرفونها (على ما أعتقد) : 1. ST elevation in aVR with infero-lateral ST depression 2. ST depression in aVL 3. Tall T in V1 which is taller than V6 4. ST depression in lead III 5. Resting U wave inversion (in leads where T wave is upright) 6. LBBB with +ve Cabrera’s sign 7. LBBB with +ve Chapman’s sign 8. Pseudonormalization of T wave #ECG
